continue his policy. The insurance costs for this type of policy increase...

policy anniversary. The premium is flexible on universal life policies, meaning
as long as there is cash value in the policy, the premium can be paid at any time. Annual
reports were mailed showing a steady decline of the cash value when
sufficient premiums were not being paid into the policy. When the cash
value depleted, [redacted] needed to pay the full premium to continue the
coverage. His policy has now lapsed without value. [redacted] may apply
for reinstatement of his policy. Normal underwriting review will apply.

I have spoke to Columbus Life on the phone and they stated to me that their computer automatically takes the money from my account. they do not have to notify me of this, but she also stated that she did not know if they were braking any laws by doing so. I find this a extremely poor way to do business when a simple letter in the mail would have stopped this.

Their admitted to me on the phone that I was never notified, they should check their own recorded phone lines. They didn't even address the fact that they drained my checking account and caused it to bounce. I can tell by their response that they could care less what they did

Review: I had submitted a complaint back in September to Milwaukee Revdex.com this was forwarded incomplete to Illinois Revdex.com. I response letter to this case by [redacted], advising me this case was close, my complaint was assigned case #[redacted]. On August 30th, 2014 I purchased 2 Happy Meals from the McDonald's on 191 W Layton Ave in Milwaukee 53207 (Store #976) to take home to my kids and when I give them to my children, they were incomlete, one of them had three fries in the bags and the other was only half full and was missing the chocolate milk, when I called the store they asked me to go back I then said what about gas? and he replied I'll give you to Happy Meals for a future visit. When I got tho the store the supervisor said I dont know who you spoke to but Im only giving you a bag of fries and to call 1800McDonlads for the rest.I called the 800 and I was told that they would send me a certificate for 2 Happy Meals that they were sorry and I would receive them in 10-15 days business days, which I never received. Again, I called I was advised it was on the way and to this day I never received it. I want to make my point clear, it is quite unpleasent to have believed that by picking up a quick lunch for the kids that should of made my day run smoother turned to be annoying, one because the kids found near to no fries and seconde to have such poor ce and care. I happened to pay attention to see what the kids were talking about, but how many times has this gone unseen by others?Desired Settlement: I would like to see my complaint resloved by having the fries and drink replaced and the 2 Happy Meals as offered as well as something extra for the inconvininece and the unnecesary traveling back and forth.

We just received the complaint listed in the subject line from a [redacted]. We were unaware that she didn't received the coupons she requested. The 800 number is an outside company they do not send coupons they forward the complaint to the supervisor who makes contact with the customer. When the supervisor contacts the guest they come to an agreement on what will happen next.We have sent out another coupon today to fix the requested coupon.
