04/17/[redacted] presented in office April 10, for care of a "chipped tooth"Patient was last seen in office in August of at which time she failed to show up for an appointment for diagnosed problems and subsequently did not make any payments for a bill totaling $ [redacted] was told that she would need to take care of bill before being seenShe made payment for total billUpon interview, she related that since she had sought care at another dental office and had several teeth extracted and dentures fabricatedthen told her that she would need to return to the office of record for any care and was not comfortable treating her any further considering the history of unpaid bill and her choice to seek care elsewhereShe related that the other office was too expensive and that she wanted me to take care of the problemTo avoid any further unpaid bills suggested that she should return to the office where the extractions were doneAdditionally, I did not feel as if she really had trust in my abilities and told her that would not be resuming any care for herShe became enraged and left the office saying she would never come backApproximately minutes later, her husband calledapologized that the situation had happened and he expressed understandingIn the background, she was yelling and threatening to turn me in for unprofessional behaviorI do not believe that truly was unprofessional but rather firm in refusing to resume care for a patient that had a history of unpaid bills, non-shows for appointments, and sought care at a different officeFinally, there was no charge for this visit nor any insurance claims filed.Respectfully submitted:Jerome P

told that she would need to take care of bill before being seen. She made payment for total bill. Upon interview, she related that since 2014 she had sought care at another dental office and had several teeth extracted and dentures fabricated. then told her that she would need to return to the office of record for any care and was not comfortable treating her any further considering the history of unpaid bill and her choice to seek care elsewhere. She related that the other office was too expensive and that she wanted me to take care of the problem. To avoid any further unpaid bills suggested that she should return to the office where the extractions were done. Additionally, I did not feel as if she really had trust in my abilities and told her that would not be resuming any care for her. She became enraged and left the office saying she would never come back. Approximately 30 minutes later, her husband called. apologized that the situation had happened and he expressed understanding. In the background, she was yelling and threatening to turn me in for unprofessional behavior.
