MD.com

This website creates physician profiles using, I think, public domain information. I have tried on numerous occasions over the past 6 weeks to complete an online request to request changes to my profile since I recently relocated and much of the information is accurate. Unfortunately, the online request form does not work and only produces a blank page when I try to submit a request. I also tried to send MD.com an email through their web form, but have never received a response. Since the current website is out of date, I am concerned that patients are being given incorrect and inaccurate information.
