On January 28, 2016, the complainant, [redacted] M ***, requested a compounded prescription that was issued by her physicianThe physician's instructions were to "apply gram topically" and the prescription was prepared and dispensed to the patient with pharmacist consultation regarding the use of this productIn August of Ms [redacted] again requested and received a refill of the same prescriptionAt no time, either in January or August, did Ms [redacted] question her physicians directions of use or the directions contained on the prescription labelIn late September Ms [redacted] returned an empty container demanding a refundAt this time Ms [redacted] was informed that had she contacted us either in January or August and indicated that she was unsatisfied, we would have accommodated and dispensed correctly on two occasions and she used all of the contents, she was not entitled to a refundReceipts are provided on all prescriptions however, receipts are not required for refunds

this product. In August of 2016 Ms. [redacted] again requested and received a refill of the same prescription. At no time, either in January or August, did Ms. [redacted] question her physicians directions of use or the directions contained on the prescription label. In late September Ms. [redacted] returned an empty container demanding a refund.
