out a Continuity of Care form, we would submit form via fax to [redacted] on the behalf of the patient.  This was the agreement between [redacted] and BHS Physicians Network until the end of year. We inform patients that we are now out of network and had the patient fill out the continuity of care form. On February 01, 2017 after I received complaint via mail, I spoke with the [redacted] representative Ref#[redacted] at 1-[redacted]8. After the representative researched on her end. She states that the continuity of care form was VOIDED by [redacted] because the patient had requested approval for date of service October 31, 2016 . I informed the representative patient had not been seen by Dr. [redacted] on that day. We had to submit continuity of care form for date of service for November 15, 2016 and November 22, 2016. I was instructed to have patient call and appeal using ref#[redacted]. I have resubmitted the original continuity of care form to [redacted] with letter of appeal with medical records from the dates of service. The patients account has been placed on hold until we can get this m[redacted]er resolved.
