Thank you for bringing this matter to our attention. Ms*** was a patient of our practice. Because we had not been notified that she had changed practices, she remained on our patient list and was notified about recent changes in the practice. It was certainly not our
intent to continue sending communications to Ms*** if she no longer wished to receive them. We did receive Ms***’s request to discontinue contact, and we immediately ceased contact. There were no further communications, except for a final letter that confirmed the end of the physician-patient relationship and providing alternative providers for care, if needed.
