Review: Credit balance of $203.08 on my account #36210 for date of service 5/4/2015 due to overpayment made at time of service to Dr. D[redacted] MD. I have been requesting this refund since 7/20/15. I have left numerous messages daily since 7/20/15. No feedback or refund has been received to date.Desired Settlement: Refund of $203.08 ASAP
