incorrect contact information for our officePlease note that we can be reach via email at *** or ***If you wish to speak to someone directly we may be reached at ###-###-#### or ###-###-####We take patient complaints very seriously and will address all complaints in a timely fashion.We have already addressed *** ***'s concern with her insurance company and will provide you with the same information.All prospective patients are required to read and attest to our Patient Rights and Responsibilities information in addition to Completing patient registration informationThis patient was no exception, The Patient Rights and Responsibilities document (attached) clearly explains the “Missed Appointment" protocol*** *** was very much aware of what was required when an appointment is missedThe patient alleges that the copy she received is "different" from what our administrator provided to herOur response is twofold:
She received the appropriate documentation and attested by her signature to the rights and responsibilities document,
In the event that she felt that she did not receive appropriate documentation, our office has made extraordinary accommodations so that our patients has unlimited access to the most current information without having to request information from our staffPlease find attached the very first page of the “Patient Registration Form” where *** *** attested to the following: "By my signature here, and my initialing the sisheet with each visit, acknowledge that I have read the Patient Rights and Responsibilities document and agree to bind myself to the terms and conditions contained therein (including updated information)”,Additionally, we have, in our patient lobby, a binder clearly labeled, in pica bold black print “Patient Rights & Responsibilities “please take oneThis binder has a page protected master copy of the document as well as several copies for patients to take if they so chooseThis binder is completely accessible to the patients without having to request information from our staff.contact us by email atmmhc staffggmail.com, by phone ***; by fax ***
Moreover, in an effort to resolve this patient’s complaint, she asked for the opportunity to pick up copies of the Sisheet as well as the Patient Registration Form that she signed and initialedShe agreed to come in to obtain the documents, but to date she has not shown up,In closing, we conduct business in a transparent and coherent fashionIf you need more information to resolve this complaint, please feel free to contact our office for an immediate response.Sincerely,
