This is regarding complaint ID *** from *** *** The office had a change of ownership on February 5, so I am going from the records in the computer system I have them attached to this email also. I show the patient was seen in the office By Dr *** ***
on November 5, for an exam and xray of tooth # Upon the exam it was diagnosed the patient needed a crown on # The patient had a coupon of $of because he was a transfer patient from Dr *** P***, Dr *** purchased Dr P*** office and merged the two practices into his Mentor location The patient did not pay anything out of pocket on November 5, 2012. The patient returned to the office on November 7, to have the crown prep done, at the time Dr *** also needed to do a Buildup on the tooth to have enough tooth structure for the crown to hold on to The patient paid out of pocket $ And his insurance was billed for the rest. The patient returned on December 12, to have the permanent crown seated It states in the notes he was happy with the look and feel of the crown Why is this now a problem years later?
