I have an accidental inaccuracy in the report I submitted yesterday, which I wished to correct. Dr. [redacted] DID prescribe a medication on the first visit, which proved ineffective. For the dates/visits in question I was working a second job that required me to be away from home 3 nights a week, and...

primary childcare responsibilities. I've been dealing with the Clinic regarding billing, but my wife did the face-to-face visits and most actual care.
