The Veteran's Administration Community Care Program sent me for a test to Maritime Radiology PC at *** (now ***) in Ellsworth, ME. The company incorrectly billed me instead of the VA, who ordered the test and gave an authorization to the radiology department the day I took the test. I told Maritime Radiology (Meridian) in a call soon after they sent me a bill. Meridian Financial refused to remove the bill from my name. I called the VA Community Care Program who said I should never have been billed and who called Meridian Financial to take ownership of the bill. Afterward, when I called Meridian about removing the bill from my name and credit, they refused until someone paid them. This is downright fraud (when you call Maritime Radiology PC's number, you get Meridian Financial, which is a collection agency) taking advantage of a 100% disabled veteran on a fixed income. This needs to be removed from my credit report and this company needs to be corrected for their fraudulent actions which may be happening to people everywhere this collection agency does billing.

I have contacted this company previously in reference to two accounts they continually furnish on my credit profile. I requested that this company validate both of these accounts by providing me with a contract or documentation bearing my signature. They failed to do so. In response to my request for account number *** they stated they received an Explanation of Benefits from *** that indicated I was responsible for $95.48 but they failed to provide such documentation. Also I am unclear on why there would be a balance *** is a state medicaid program that pays 100% of their allowable contract rate. Please provide the explanation of benefits as well as the insurance denial letter.

In response to account ***his company responded to my request that information was mailed back in 2013 I am not sure what address was used and I have never received any documentation. If you want to argue this fact please provide me with a proof of delivery that was sent and shows confirmation of receipt. This company also states that on 2/26/14 they called *** and was advised that the claim was denied pending accident details. I have not the slightest idea what that even means. Please provide me with the denial information and since then has there been an update?

Not only are neither of these accounts valid I filed a bankruptcy for the dates of services and believe these accounts should be listed.

Please provide requested documentation and verify the bankruptcy covered these dates of services.

Good Morning,

In regards to the $95.48 balance due on the patient's account, this is their deductible per the insurance company. Every insurance company always provides a copy of the Explanation of Benefits to the patient, it is the law and insurance companies are required to do so. The patient at the time did not have Medicaid coverage, therefore, there is a balance due per*** At any time, the complainant could contact her insurance company to request this information since they already provided it to her. We absolutely have no problem sending her a copy of this EOB but since it is so old, we will have to pull a copy of the EOB from storage but it will take at least a week to get it so we will need more time to supply this to the patient. Again, all she would have to do is call her insurance company to request this information since it was her visit and they previously supplied this to the patient.

As far as the balance of $691, the patient received numerous statements on 06/03/13, 07/14/13, 08/05/13 informing the patient that there was a balance due and if she did not contact us she would be subjected to collections. Her insurance company, *** stated they were not paying the claim because they too were waiting for a response from the patient/complainant regarding additional information she needed to supply to them. She did not respond to the insurance company either. Again, if she contacts the insurance company this could all have all been resolved. As far as confirmation of receipt we sent the numerous letters by regular mail to the address the patient/complainant provided at time of service. We will absolutely pull the EOB for this account also but again since it is so old we have to get it from storage which will take about a week.

We are not the custodian of the medical records, therefore, if the patient/complainant would like her visit information she is more than welcome to contact *** for the account from *** and *** for the *** visit. They will supply her with her records no problem at all.

If the patient/complainant can supply us with her bankruptcy information showing *** and *** as a debtor, then we will absolutely remove this from her credit immediately no problem at all.

We work very hard on a daily basis with our client's patients in order to resolve their outstanding balance so it does not put an undue burden on them.

This company which I hold no contract with nor have received services from reported a collection account against my SSN in the amount of 100.00 with *** & *** credit reporting agencies. I requested verification of the debt and account however the business failed to provide adequate proof. Considering this business does not have a contract with me for goods or services they have provided nor have they provided adequate proof I am not obligated to pay for the alleged debt.

On 09/14/216 this patient received radiology services at ***. She received a statement on 10/20/2016 informing her that the services performed are not covered with Maryland Medicaid and that she is responsible for the balance. For the next 4 months we sent notices and dunning letters informing her of this balance and the action we were going to take. We never received a response from the patient in regards to this visit, so she was reported to the collection agency. On 07/17/2017 we also responded to the dispute she filed with ***.

I do not have a signed contract nor any contract with Meridian Financial Management. I have never done business with this company. I am not liable for collection from this debt from ***. I am not responsible for this debt with Meridian.

Good Afternoon, *** has a balance for his visit to the ***, which his insurance paid the majority of his visit but left a deductible balance that he is responsible for. He was sent numerous statements and 2 dunning letters informing him of his balance and the reason for his balance.

I have requested that this company validate a debt that is alleged that is owed to them. They have not validated the debt and removed one item from my credit report. They did not remove the other but now they have added the account back to my credit report without notification or validation of this debt. I am requesting that they remove this alleged debt from my report. I have not gotten a written response from this company about any debt they claim is owed. I requested validation in January and even requested it to be verified several times through *** and I have not received any information.

received radiology services in 2014, was sent to collections in 2015 and has remained there ever since, this has not been removed from her credit and then “put back on”. She was sent a couple statements and 2 dunning letters informing her she was going to collections. We have responded 3 times to her disputes on *** dating back to December of 2016 and on July 12, 2017 sent her a statement of her account and a letter explaining these charges.
