I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below1.First thing my insurance card says $for ER visit2.Next I never signed up for $deductibleIt never was any plan offered that high deductible3.Paystubs show amount I paid per paycheck for $deductible4.Why is the plan called (HSA) if you do not get any money back /most plans that have high deductible you get money at end of term if you don't use itNever went to MD or ER (scam) in 2014,where did all that money go? *See attached Employee benefit contribution sheet Regards, [redacted] ***

Meritian has not paid our medical bills due to waiting for a phone call from us to verify we did not have a 2nd insurance We do not have a 2nd insurance They should really check this information out when we sign up for open enrollment instead of NOT paying the bills and having collection agencys calling all hours of the night Effective from 1/1/to presentpaperwork faxed

my complaint. For your reference, details of the offer I reviewed appear belowI have been told by Meritain employees on multiple occasions that my
claim was set to be paid and they had everything they needed! I
attempted to attach a phone conversation that I recorded to this but it
isn't an accepted file type. I have recorded all of my calls with
Meritain and if you provide me with an email I would be more than happy
to send the audio files to youEach time I ask for the reps name,
number and contact info so they clearly can tell who is responsibleI've
contacted the hospital IHC and they also have told me repeatedly that
Meritain has made outlandish excuses in hopes of delaying paying their
bill (I have these calls recorded)IHC also says that they are almost
always unresponsive to all of their requestsTHIS CLAIM WAS SUPPOSED TO BE PAID ALMOST TWO YEARS AGO!! I
FOLLOWED ALL OF THIER GUIDELINESI WAS IN CONTACT WITH THEM REGULARLY
I MET 100% OF MY INSURANCE DEDUCTIBLE AND THEY WERE RESPONSIBLE TO PAY
MY CLAIMI FILLED OUT ALL THE FORMS THEY EVER REQUESTED. MERITAIN
IS ACTING IN BAD FAITH AND THEY'RE RESPONSIBLE TO PAY THIS CLAIMTHEIR
OWN REPRESENTATIVES OFFER CONDOLENCES TO ME WHEN THEY LOOK AT MY FILE
AND HAVE NO CLUE AS TO WHY IT STILL HASN'T BEEN PAID. MERITAIN, PAY MY BILL!!!!! Regards, *** ***

Please be advised that the individual is a participant in an employer sponsored self-funded health plan (the Plan), Which is governed by ERISA. The Plan is the IASIS Healthcare LLS Welfare Benefit Plan. Meritain Health, Inc. (Meritain) is the third party claims administrator for the Plan. Meritain Health is neither an  insurance company nor the fiduciary for the Plan.  I have researched this matter with our Operations Department and have been advised that this claim was paid based on the provider's contract with the Altius Network and pursuant to the Plan's Medical Schedule of Benefits. I have enclosed a copy of the Explanation of Benefits.
