Dear Ms*** This letter is in response to the complaint you filed with the Revdex.com of Central TexasTheir letter dated September 24, was received in our office for review and response on September 29, 2017.Keeping policy owner information secure and private is one of our top prior tiesIn compliance with state and federal privacy laws, we are required to restrict access to policy information to the policy ownerWe are allowed to disclose information only as authorized in writing by the policy owner, or as otherwise permitted by law. Because we have not relieved your written authorization, we cannot provide a detailed response to the Revdex.com regarding your complaintYour concerns are being addressed directly with you. Our March 15, letter was mailed to you regarding your future billing options on your policy, as premiums were no longer being paid through payroll deductions with University Healthy SystemsMonthly direct billing notices began for this policy and your additional three policies effective March 15, 2017, for the April 1, premium due. Subsequent monthly premium were billed on all four policies.Our June 16, letters notified you that your children's policies terminated for a non-payment of premiums and your husband's policy was place on extended term insurance expired on July 25, Because your policy had sufficient cash value, the outstanding premium were pad by automatic premium loan, as this was the Non-Forfeiture Option selected on your ApplicationThe May, June, July and August premiums were paid by Automatic premium loan when the outstanding premium remain unpaid. On July 19, 2017, we received your handwritten notice to "cancel Policies; on our previously mailed Insurance Bill - Final Notice for the April premium dueOur July 20, letter was mailed to you with a Request for Cash surrender form to be completed, so that we could issue you a check for the cash surrender valueUntil a properly completed Request for cash surrender form is received in our office, the policy remains in force with premiums due.We do not show receipt of a signed Request for Cash Surrender form prior to your telephone call to our office on September 19, At that time, you expressed your concern that we had not canceled your policy and that e had paid the outstanding premiums by Automatic Premium LoanWe explained that we would need a properly complete form to process the termination and issue a check for the policy;s cash surrendered valueWe mailed and addition Request for cash Surrender form to you that same day.On September 27, we received your completed Request for Cash Surrender formThis form was being reviewed at the time we received your complaint from the Revdex.com on September 29, It was noted that you did not provide your social security number on the form to verify with our recordsUnder circumstances, we would not be able to accept this form without the required information. While Texas Life followed our standard procedure for requesting the cash surrender value of a policy, we are willing to make a one time exception to accept that form you signed and honor your request to surrender the policy as of JulyThe enclosed check for represents the cash surrender value as of July The policy is no terminated and provides no further insurance coverage. We show our letter dated February 7, 2017 was faxed to you in response to your February 1, 2017 letter.This letter will serve to inform you that keeping policy owner information secure and private is one of our top priorities. In compliance with state and federal privacy laws, we are required to restrict access to policy information to the policy owner. We are allowed to disclose information only as authorized in writing by the policy owner or as otherwise permitted by law.
