They have said nothing new in their response. I was informed I would be charged $30-$40, and never told about a deductible. I was asked to sign a form which acknowledged that I was informed of my insurance benefits.

Merchant response is simply untrue.  When I arrived I was informed explicitly that the visit would be 20% copayment only, and was not told about a deductible.  I was informed that that copayment would be $30-$40.  I was given a form that said only that I was explained my benefits.  I am happy to pay the actual stated charges, which is up to $40.
Sincerely,

Review: I had physical therapy on 1/**/**, and the financial person ([redacted]) called my insurance company and then informed me that my visit was covered, and I would be charged 20% coinsurance, which should be $30-$40. She gave me a sheet of paper to sign, saying how many visits I had and that I would be responsible for coinsurance. I was subsequently billed for the full amount of $367 (for a one hour session!!!) and when I called to complain, I was told that it was only covered after I met my $1500 deductible, and the billing department refused to reduce the charges to what they had informed me on my initial visit. I sent in a check for $40 to cover the stated charges, which they have not yet deposited, and they are continuing to bill me for $367.Desired Settlement: To accept my $40 payment as full payment for services rendered, and stop billing me for charges never agreed to.
