I mad a dental apt with office twice on a roll I was told I was told I was out of network on the first visit I drove miutes for my apt with my family then employee called me and told me that I was in network and It was ok for me to get ex ray and cleaning done which I did , but then when I checked the front desk the other rep told me that I was being charge out of network which, then a week later I went online to check for my claims for my insurance I was charged dollars which I wasn't aware I asked the office for them to reimburse me for the incovince and the dollars that I was charge for they declined , I need my money back

The above mentioned patient was given treatment which consisted of an oral prophylaxis (dental cleaning) and fluoride treatment. Patient claimed that she was not told about the fluoride treatment which was $The fee was waived for the fluoride treatment and her cleaning was paid in full
by her insurance. A copy of the ledger is enclosed for your review
