Thank you for your recent inquiry regarding [redacted] ***We offer the following information in response to [redacted] ***’s complaintA complete Dental Insurance Policy, including the ID card, was mailed to [redacted] at [redacted] *** [redacted] on December 8, The ID card provided the number for Customer Service should [redacted] have any questions regarding his benefits or need to locate a network providerThe first page of [redacted] ***’s Policy states in bold print, “Notice to Buyer: This Is A Limited Policy – Read It Carefully”The Schedule of Benefits outlines the limited services/codes that are eligible, which include Diagnostic Evaluation Services, Diagnostic X-Ray Services and Preventive/Phrophy Services (teeth cleaning)The Policy further states, “We will not provide any benefits for any loss caused by or resulting from: any portion of a charge for any service not listed as a Covered Expense in the POLICY SCHEDULE / SCHEDULE OF BENEFITS.” The services in question, [redacted] and [redacted] are not listed in the Policy Schedule and are therefore not a covered service [redacted] and his dentist were made aware that this was not a covered service on multiple occasions: January 25, – Explanation of Benefits to [redacted] denied the charges for [redacted] as not coveredFebruary 14, - Explanation of Benefits to [redacted] denied the charges for [redacted] as not coveredJune 28, – [redacted] contacted Careington for benefits [redacted] was informed the policy only covered the services shown on the Schedule of BenefitsJune 16, - The dentist submitted a Pre Determination of Benefits request on June 16, for service codes [redacted] and ***The dentist was advised the services were not a covered expense under *** ***’s policy, prior to services being rendered on August 17, Based upon the information outlined above, our position remains unchanged; the [redacted] and [redacted] charges incurred by [redacted] are not covered servicesWe trust this answers any questions you may haveIf we can be of further assistance, please do not hesitate to contact me by phone at [redacted] , ext [redacted] or via email at [redacted]

Mid-West National Life Insurance Company of Tennessee is in receipt of your February 7, letter regarding the above mentioned case number.While we understand the complainant's frustration, please understand that we are only able to providelimited information to the complainant in this situation since she is not the policyholder or the beneficiaryUnder the terms of the life insurance policy, since the named beneficiary was deceased prior to the date that the policyholder died, the policy looks to the estate of the policyholder to determine the appropriate distribution of life insurance proceedsThe life insurance proceeds were not payable to the estate of the beneficiaryAll life insurance proceeds have been paid in accordance with the terms of the policy.If you have any questions, please feel free to contact me at (***) ***-*** or via email at ***.com if you have questions or need any additional information.Sincerely,*** ***

I am rejecting this response because:
Both the insured and the beneficiary are deceased and I am the daugther of the beneficiary, and niece of the insuredSo who's privacy is the insurance company protectingThe companies's reply is pretty much the same reply previously mailed to meIt appears to me that the COis attempting to avoid a payout on the policyI will again state that certificates of death were mailed to the company previously

ID card provided the 800 number for Customer Service should [redacted] have any questions regarding his benefits or need to locate a network provider. The first page of [redacted]’s Policy states in bold print, “Notice to Buyer: This Is A Limited Policy – Read It Carefully”.
