the office sent me a written note to tell me that the balance was for 3/3/I sent them the prove of payment for that day, now when I talk to them they told me it was for 1/26/I am still waiting for a written statement that I own them for 1/26/15, because they have change date in the pass

I have received your letter regarding Ms*** ** *** disputing for an amount of $from her bill Ms*** has been seen in our office several times by DrDock W*** and she have a co-payment of $every time she visits our office due to her insurance policy For the date
of service of January 26, she was in our office for Urodynamics that was performed that day,the claim was submitted to her insurance (United HealthCare) There was a total charge of $3,for that date of service and her insurance paid $there was a patient responsibility of $due to her deductible For the date of service of March 3, she paid $for her co-payment but that amount got applied to her previous balance last January 26, she also have a patient responsibility of $on her March 3, visit which sums the total of her balance up to $which states on her most recent statement. I've attached a copy of Explanation of Benefits that came from her insurance company with this letter if you have further question please call our office at any timeSincerely,
