July 27, 2015Dear [redacted] ***,This letter is in regards to a recent complaint you received from a patient we provided anesthesia services to on October 23, The patient presented to the facility we service ( [redacted] Medical Center) for a surgical procedure Upon admission, patients are asked to provide information facility to forward our bills to the patient, however this patient's information was recorded incorrectly and therefore we sent the bill to the wrong address Once we have forwarded billing statements to a patient and do not receive a response, it is then forwarded to collections Unfortunately for this patient, she never received a bill and then discovered this outstanding bill when checking her credit report The patient contacted our office on May 11th and we immediately removed this from collections We apologize for any inconvenience this may have caused her we hope that she will consider Middletown Anesthesia Group and [redacted] Medical Center should she need any future treatment If you have any questions please do not hesitate to contact our office at the number listed above Sincerely, Erich EPresident
