Midwest Convenient Care received a letter from the Revdex.com filed by *** *** regarding patient *** ***' 07/23/visit to our office. The process in our office is to send statements allowing the patient days to respond to the balance. All patient
statements are sent to an outside vendor for processing.*** *** came to our office and spoke with a member of our staff and stated he had not received a statement regarding the balance. Our practice management system gives us the history and date of all the statements sent to our patients and it showed indeed two statements had been sent to the patient. We would have had no reason to question.We decided to do an internal audit of what happened. We send all statements to our billing vendor, who in turn sends electronically to the vendor to process the statements. In the process of transmission, it appears there was an issue of the statement file and a statement was not sent.The collection company was notified immediately, and the patient was removed from collections.Our office has spoken with the patient and due to this being an error in the statement processing, we have returned the patient's payment back to him and the balance has been adjusted.We apologize for the inconvenience this has caused
