I read through the complaint (which [redacted] informed [redacted] in our office earlier this week was coming) and will clarify a few things.· The first policy we wrote on their business was spring of We wrote a policy to cover their liability and equipment At that same time we provided them with a quote on Workers Compensation based on potential employees – they ( [redacted] & Alan, her husband) did not wish to pursue at that time· We added an auto policy when they informed us the needed.· Their business liability policy is non-audited, which means we (the company and the agency) isn’t privy to payrolls, employee counts, etc That is of course run through the Department of Workforce Development in Wisconsin.· They notified us late September of that they needed Work Comp insuranceo We went to Erie Insurance to provide coverage since that was who wrote the auto & liablity Their stance when finding out that an existing business did not have coverage they would not do We worked with our underwriter to offer the coverage to O What a Day to avoid having their coverage placed in the state Work Comp pool which would not have allowed for a combined billing and less flexible billing options in general.o Coverage was secured for Work Comp 10/14/14· O What a Day last made a payment on their policies December of Erie sent out notices in (I don’t have all the dates) that were not paid Their auto canceled 4/24/and the Work Comp back to 3/26/15· In June when [redacted] requested to see if we could backdate coverage back to or (more than a couple years): that was not something Erie could do, especially since the policy had lapsed for non-payment back in March 2015.· After I talked to [redacted] she made it clear she was going to move her insurance elsewhere which we agreed was probably in her best interest She asked for copies of old policies and loss history multiple times which we provided immediately (within a day) each time she requested something While it wasn’t necessarily me personally responding to her requests we have an office staff of of us we provided information requested multiple times.· The fact that O What a Day Café is in the position to file a complaint and we need to respond is disappointing – this is obviously not what we want to be spending our time responding to or more importantly our client feeling the need to file a complaint which is not helping them grow their business and adds headaches As a small business owner myself I understand these things don’t help anyone Customer’s Statement of the Problem:We contacted Midwest Insurance Group in the Spring of to purchase business insurance to cover our new business that was opening it first store frontAfter speaking with the agency, I remember distinctly asking if I had everything I need to run my small business safely and in compliance with all regulatory agenciesThe confirmed that as an insurance agency and professional adviser, they would make sure I was coveredI began paying my policies immediately and assumed all was wellOur business began to grow and we expanded added and expanded to a new location, we added on a insurance policy for a commercial auto policy for our new delivery vanWe began paying our policy believing we had all the coverage and protection we neededIn October of 2014, we found out that we did not have Workers Compensation insurance, we were shocked, we remember discussing employees, etcSome how, it fell through the cracks? We contacted Midwest Insurance immediately and added on the Worker's CompensationThey weren't sure what happened, and it was very hard to get any feedback from the ownerAt that time, they NEVER mentioned to us that we could be in any kind of trouble for not have Worker's CompensationWe continued to work hard, very hard, every day in our small business, trying to keep our staff employed and our business open in WisconsinWe have been paying for Midwest Insurance services, Accounting Services and Payroll services since assuming, we were free to pursue our dream of operating our small business in full complianceIn the end of May we received a $18,fine from the State of Wisconsin Worker's Compensation department for not having the insurance from May to October After long discussions with our state of Wisconsin agent, we were informed that we should have been protected by having so many advisers, an insurance agent, accountant and payroll services, someone should have noticedAfter trying to talk with Midwest Insurance, they didn't have a response, they didn't say sorry or care, when I asked him what his role was for our business, he said insuranceAfter asking for help and a resolutions, such as a back dated policy, they never contacted me backAfter contacting him again, he claimed that he doesn't follow through or communicate with his clients, it's their jobAfter meeting with new insurance companies and accountants to get support for our business, change services and get assistance with this fine, all of them have said that this was Midwest Insurance ResponsibilityI've been told that Workers Compensation is the or the ABC of insurance servicesSMALL BUSINESS BEWARE, Midwest Insurance does not have your business in their best interestThey will get you a policy and take your premiums but they do not care about you, your small business or providing any level of quality customer servicesAfter going through this, I've heard about others dealings with Midwest, they can get you cheap rates via Erie Insurance but they don't care about their clientsI've called both owners, I've spoken with other employees, I am now filing an official complaint via the Office of Commissioner of InsuranceIf you are going to take the huge challenge of starting a small business in Wisconsin, make sure you have the proper support and advisers that believe in the meaning partnersMidwest Insurance is not that Partner and when the make a mistake, neglect their clients, they don't even apologizeSMALL BUSINESS BEWARE!

We have been more than busy running the business, assuming all was well until October when another more reputable insurance agency cross quoted and informed us of the "very big problem" We have been working the business, morning; noon and night, not staring at documents When we contacted Midwest insurance in October, that would of been a great time to explain that we could be facing serious fines, upwards to $18K and to work on a solution or resolution to this issue at that time.Again, they took our money, gave us a package and that was it If they are sincere at what they do which I understand is to be providing insurance needs to their clients, this mistake is their responsibilityMany small business owners and owner/ operators would agree, as they had since this has come to light We have gotten more feedback and communication from Midwest Insurance since we submitted this Revdex.com report than we have since May when we called them in desperation about the fine We are paying for the fine, lesson learned! Small business beware, when the going get tough, Miswest Insurance Group, will not call you back, will not have their staff call you back as [redacted] stated, you will have to pursue them at all times [redacted] will say he will call you back, will look to see what can be done but he won't call, he won't care, he will blame your business.If I would have got any kind of follow through by Midwest Insurance, this could have been prevented So unless you are an insurance expect that has a ton of time available to navigate the State of Wisconsin and its insurance process, beware; Midwest Insurance likely isn’t the company for you.Regards,

See the below email string – showing I provided [redacted]e a Work Comp quote 2/23/12 based on their business plan/projections.  In my 3/9/12 email I asked again about what she wanted to move forward with…including the Work Comp or not.  She did not and when we sent application forms workers compensation was not included. The Wisconsin Department of Workforce Development (DWD) – Workers Compensation division sent notices to O What a Day Café notifying them of their Workers Compensation needs (based on cross reference with the Unemployment Insurance Dep’t which has access to payroll submission data) on the following dates (that I’m aware of):·         9/2/12·         11/21/12·         2/... [redacted] of the Midwest Insurance Group in Delafield and Wauwatosa is not telling the truth on this matter.  We were NEVER offered Worker's Compensation, when I was insured with Liability, etc it was for a retail store, there would be NO WAY it could have been run alone.  Note the initial application that states we had 6 employees on my original 2011 application.  I opened with employees and now learn that it's the law to have Workers Compensation.  It was [redacted]'s job as an insurance agent to protect our company, he did not!  He is lying to cover up his mistake, his errors and omissions.  I total him from the beginning I wanted to have what ever it would take to run a small business and he said he had us covered.  Again, when we did find out more than 2 years later, he never mentioned we could get in trouble.  All things as an insurance agent, he knew and kept from us.  [redacted] is also lying about us agreeing to take our business somewhere else, of course that is a given, but it was not mentioned or discussed. I only spoke to [redacted] (who is nice) because [redacted] was not confident enough to handle the issue and call back a client.  [redacted] also told me that he would call me back about trying to get a back dated policy (which was the recommendation of the State of Wisconsin Worker's Compensation Department), and again he never called back to even say no.  I continued to pursue him, and after we found out about the lapse policy, I called him again, he said again, he would call me back and he did not.  [redacted] was the only person to give me any information, not the agent who sold me the policies over the last four years.  As a small business owner that [redacted] claims he is, he doesn't really know that word.  He will be there to take your money but he won't follow through on anything else, much less call you back and try to help, but he will hide behind his staff.  As a small business owner in an industry that requires hours on end, days, nights and weekends  I have been working under the assumption that [redacted] was doing his job as my agent and protecting my company and our staff, he did not.  [redacted] is a liar, he is not trustworthy, does not follow through or resolve anything when he is wrong.  Anyone in need of business and/or  personal insurance  should avoid Midwest Insurance Group.  While you are growing your business or living your life, they won't have you covered and when something goes wrong because they didn't do their job, they will blame is on their client.  BUYER BEWARE!Once again, [redacted] has done nothing to resolve this issue, he is bad business for small business.

I've been told that Workers Compensation is the 101 or the ABC of insurance services. SMALL BUSINESS BEWARE, Midwest Insurance does not have your business in their best interest. They will get you a policy and take your premiums but they do not care about you, your small business or providing any level of quality customer services. After going through this, I've heard about others dealings with Midwest, they can get you cheap rates via Erie Insurance but they don't care about their clients. I've called both owners, I've spoken with other employees, I am now filing an official complaint via the Office of Commissioner of Insurance. If you are going to take the huge challenge of starting a small business in Wisconsin, make sure you have the proper support and advisers that believe in the meaning partners. Midwest Insurance is not that Partner and when the make a mistake, neglect their clients, they don't even apologize. SMALL BUSINESS BEWARE!

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint. For your reference, details of the offer I reviewed appear below.

[redacted] of the Midwest Insurance Group in Delafield and Wauwatosa is not telling the truth on this matter. We were NEVER offered Worker's Compensation, when I was insured with Liability, etc it was for a retail store, there would be NO WAY it could have been run alone. Note the initial application that states we had 6 employees on my original 2011 application. I opened with employees and now learn that it's the law to have Workers Compensation. It was [redacted]'s job as an insurance agent to protect our company, he did not! He is lying to cover up his mistake, his errors and omissions. I total him from the beginning I wanted to have what ever it would take to run a small business and he said he had us covered. Again, when we did find out more than 2 years later, he never mentioned we could get in trouble. All things as an insurance agent, he knew and kept from us. [redacted] is also lying about us agreeing to take our business somewhere else, of course that is a given, but it was not mentioned or discussed. I only spoke to [redacted] (who is nice) because [redacted] was not confident enough to handle the issue and call back a client. [redacted] also told me that he would call me back about trying to get a back dated policy (which was the recommendation of the State of Wisconsin Worker's Compensation Department), and again he never called back to even say no. I continued to pursue him, and after we found out about the lapse policy, I called him again, he said again, he would call me back and he did not. [redacted] was the only person to give me any information, not the agent who sold me the policies over the last four years. As a small business owner that [redacted] claims he is, he doesn't really know that word. He will be there to take your money but he won't follow through on anything else, much less call you back and try to help, but he will hide behind his staff. As a small business owner in an industry that requires hours on end, days, nights and weekends I have been working under the assumption that [redacted] was doing his job as my agent and protecting my company and our staff, he did not. [redacted] is a liar, he is not trustworthy, does not follow through or resolve anything when he is wrong. Anyone in need of business and/or personal insurance should avoid Midwest Insurance Group. While you are growing your business or living your life, they won't have you covered and when something goes wrong because they didn't do their job, they will blame is on their client. BUYER BEWARE!Once again, [redacted] has done nothing to resolve this issue, he is bad business for small business.

See the below email string – showing I provided [redacted]e a Work Comp quote 2/23/12 based on their business plan/projections. In my 3/9/12 email I asked again about what she wanted to move forward with…including the Work Comp or not. She did not and when we sent application forms workers compensation was not included. The Wisconsin Department of Workforce Development (DWD) – Workers Compensation division sent notices to O What a Day Café notifying them of their Workers Compensation needs (based on cross reference with the Unemployment Insurance Dep’t which has access to payroll submission data) on the following dates (that I’m aware of):· 9/2/12· 11/21/12· 2/26/14· 7/21/14· 9/17/14· 4/29/15 PRIVACY STATEMENT: This message and attachments are confidential and are intended only for the addressee(s).
