Milan Institute (MI), Merced campus, welcomes the opportunity to respond to the complaint, dated April 21, 2017, and filed by *** *** (“Ms***” or the “student”) who is enrolled at Milan Institute in Merced, CA In her complaint, Ms*** stated that she has yet to receive
disbursement of funds for her educationMs*** stated she is concerned about being required to pay on loans she has yet to receive MI is currently undergoing a new mandatory student file review process resulting in a delay of the releasing of funds from the Department of EducationDue to the delay caused by this process, student fund disbursements take longer than to process and postMs*** is only responsible to begin payment on any loans she has already received; once the remainder of her funding is disbursed, she will then become responsible for payment on that part of her loan Additionally, Ms*** can check the National Student Loan Data System by visiting www.nslds.ed.gov anytime for updates on her loan disbursements As soon as the funds for Ms***’s student loans are released to MI and applied towards her tuition, any credit balance left over will be released to her in accordance with federal regulations

Milan Institute (“MI”), Home Office, provides this response to the complaint filed by *** *** *** *** Milan Institute acknowledges that it is in receipt of the Revdex.com (Revdex.com) complaint that had been submitted on February 14, and was assigned an ID of 11***83,
regarding the concerns expressed by Ms*** Ms*** has alleged in her complaint that she was never a student at Milan Institute and that this incorrect information is hindering her ability to move forward with her education In her initial contact on February 14, with the Registrar at the Milan Institute of Cosmetology campus in Fairfield, CAMs*** was informed that she not showing in the schools’ database systemThe campus registrar then reached out to MI’s Home Office to see what more information could be found regarding Ms***’s situation Upon further research, it was found that between the years of 1997-1998, Ms*** attended Trinity Allied Health College in San FranciscoTrinity Allied Health College did a teach out of its programs and Milan Institute opened in the same location and began to offering Cosmetology programs after Ms*** had attended, this explains why Milan would appear on any Financial Aid or school history reporting Ms*** has no outstanding balance with MI and there is no reason as to why her previous enrollment at Trinity College would be impacting her ability to continue her education at another institute Ms*** may contact MI transcripts department at *** and request to have a copy of her Trinity College transcripts sent to her prospective school

*** Institute of Cosmetology, La Quinta campus would like to respond to Ms*** *** rejection to our response In her response Ms*** stated that there were not two instructors during the morning/day shift *** attended the morning Theory class in which there was one instructor for Theory and for State Board focus, keeping us below the ratio of instructor to students- as there were 18- students enrolled during that time*** moved to the Phase salon floor on 9/29/With 18- students enrolled, the student population only required one instructor to fulfill the instructor to student’s ratios, however additional Instructors were added for support during the busiest salon times Ms*** alleges that *** was worried because she had witnessed intimidating tactics to retain other students by means of denying request for proof of training or requiring monthly payments to terminate a contract This allegation is incorrect, when students express a need or desire to withdraw the staff will try to help the students with alternative options so that they may continue to work toward accomplishing their educational goalWhen a student submits a request for their Proof of Training they must first submit a letter of request for withdrawProof of Training can only be released once the drop calculation is complete and any additional balance is paid or they are set up on payment arrangementsUntil the drop calculation is processed, we are not be able to tell a student whether they have a balance owed to the campus Ms*** stated that ***’s supplies were received all different and that some students received more than others *** acknowledged and signed for all items received This is attached again for review The CND sculpt brush was on backorder form the Phase kit and that was released to her once the backorder was filled by our vendorThe other supplies for clients or student practice are provided based on the service or practical and are dispensed based on those factors Ms*** stated that *** was given the run around and that she tried to leave two weeks earlier On 10/20/*** went to the Education Financial Aid Advisor and the School Director to ask about a balance due if she were to drop that dayWe were unable to provide that information even after asking FA management support team directly for an estimated drop balance***’s last day of attendance was 10/21/2016. *** met with the Education Financial Aid Advisor to set up a payment arrangement for drop balance on 11/11/The School Director offered to create her Proof of Training right afterwards but she stated that she didn’t have time to wait and could she come back on Monday to pick it up Ms*** stated that when she visited the campus for a service on 8/16/she came in under the alias of *** *** and received services from a student near completion who did not know what he was doing The student who provided Ms*** service at the time had hours completing only 42% of the program, not someone you could label as “ready to graduate”On review of the client notes the School Director was unable to find any documentation that “Ms***” was unhappy with her serviceWhen there is a client complaint we offer to redo it, the Instructor then counsels the student on improving the skills needed to be successful and enters a note in the Student Information System that the student was counseledUpon review of the students notes there was nothing found to support Ms***’s claim *** Institute of Cosmetology stands by our original response and feel as we have met all our obligations to *** outlined our Enrolment Agreement contract Attachments include the following: Student Population and Staff Schedule, Phand kit list of items received and signed for along with tablet sign off, student notes documenting conversations between Student, School Director and Education Financial Aid Advisor

Milan Institute of Cosmetology (MIC), San Antonio campus, would like to respond to the complaint filed by *** *** (“Ms***” or the “student”) who was a student of Milan Institute of Cosmetology in San Antonio TXMilan Institute of Cosmetology is in receipt of the RevDex.com (Revdex.com) complaint that was submitted on March 27, and was assigned an ID of ***, regarding Ms***In her complaint, Ms*** stated that she has been unable to resolve a negative credit balance she previously had with her Milan Institute account despite her efforts to submit proof that she paid off the debt through a collection agencyUpon further research into Ms***’s account it was found that her debt to MIC San Antonio was written off by the campus for nonpayment and her outstanding debt was sent to a collection agencyMs*** submitted proof of payment to MIC home office and the hold on her transcript and hours was releasedTo resolve this matter and ensure that Ms*** can continue her efforts to finish her education, MICs compliance coordinator mailed out a copy of her official transcript to her school of choice on 3/28/MIC has also updated Ms***’s account information in the state data systemMIC spoke with Ms*** on 3/28/to inform her of this resolutionMIC considers this complaint with Ms*** resolved

[redacted] Institute of Cosmetology, La Quinta campus would like to respond to the complaint filed by [redacted] on the behalf of her daughter [redacted] De Los [redacted] who was a student of [redacted] Institute of Cosmetology in La Quinta California.   Ms. [redacted] stated that [redacted] Institute provides...

inadequate educational services: for instructor to student ratios.  Our instructor to student ratio, which follows the state mandate, is 1 instructor for every 30 students. During the three weeks, [redacted] was on the student salon floor  there were 27 active students with one to two Instructors and occasionally three Instructors assisting during this time. [redacted]’s group came on to the student salon floor (9/29/2016) and in anticipation of having more students on the student salon floor, an additional experienced Instructor was hired (8/26/2016) and added to the salon floor schedule to provide adequate assistance.   Ms. [redacted] claimed that [redacted] Institute of Cosmetology is not properly preparing students for the state board licensing exam. [redacted] Institute of Cosmetology’s La Quinta campus had a student licensure rate of 98% for 2015 in accordance with our accreditation (NACCAS) standards. [redacted] ‘s last day of attendance was 10/21/2016 –only allowing her to attend three weeks or three state board preparation classes.   Ms. [redacted] alleges that [redacted] Institute of Cosmetology is unable to teach left handed students and does not teach proper techniques. When the phase 1 and Phase 2 kits are dispensed – [redacted] provides left handed shears for each kit. [redacted] received the left handed tools when both of her kits were dispensed. Unfortunately, [redacted] did not voice her need for additional left handed instruction to the Instructor or the school director. [redacted]’s instructors have assisted many left handed students successfully.  Our Instructors teach several different ways on how to adjust body and hand positioning for left handed techniques.   [redacted] Institute of Cosmetology has been unable to validate Ms. [redacted]’s claim of receiving a dissatisfactory service from a student near graduation who did not know how to set up or do the service properly. We are able to validate that the only appointment of record for Ms [redacted] – was canceled for one student and rescheduled for a manicure from [redacted] on 10/14/2016.    At that time, [redacted] was not near graduation, she had only completed 16 weeks of a 53-week program. There are no notes on record of dissatisfaction or complaints from Ms. Riveria in regards to the service she received from [redacted] that day.   When [redacted] informed her School Director that she needed to withdraw from [redacted] Institute of Cosmetology, the reason she gave was because her mother was experiencing financial difficulty and decided to move to Riverside for cheaper housing as well as a better job. [redacted] also shared she really did not want to quit because she enjoyed coming to school but she lived with her mother and she was concerned for her mother to continue to pay for the school. [redacted] said that she tried to work so that she could to help her mom out. [redacted] stated she thought continuing to attend would put a financial burden on her mother and dropping from school would remove that burden.   Settlement Response: Ms. [redacted] states that her daughter deserves a refund because the only things she learned while attending [redacted] Institute of Cosmetology were a basic haircut and manicure because there isn't enough staff on floor to show more. While [redacted] was enrolled at [redacted] Institute of Cosmetology, she completed all of the practical requirements, assignments and tests to move from the twelve week, 360 hours – Phase 1 portion of her education to the Phase 2 student salon floor for 3 weeks. The Phase 1 portion of the education includes Instructor demonstrations, along, with student practice following curriculum on techniques in the areas of Skin, Nails, Haircolor, Haircuts, etc.  . For the three weeks that [redacted] was on the student salon floor (Phase 2) there was minimum of one Instructor technique demonstration per day for students to observe and follow with practice.   Ms. [redacted] states that [redacted] never received the textbooks and additional items [redacted] Institute of Cosmetology is trying to bill for. Upon review of [redacted]’s student records [redacted] Institute is able to prove that [redacted] signed for and acknowledges full receipt of both portions of her cosmetology kit which includes among other things – electronic text books and a tablet.    Attachments include the following:  Ms [redacted] client history in our software for the student salon, Leloni’s Theory Grade sheet,  Phase 1 assignment curriculum check off sheet, a grade card reflecting Practical procedures [redacted] Completed while enrolled, Ph1 and 2  kit list of items received and signed for along with tablet sign off, student notes, student withdrawal letter, and  our Education Financial Aid rep asking a follow up question for the student in regards to dropping for financial reasons.      Due to the information provided above and the confirmation she signed for her kits and supplies, we are unable to refund any money owed to the school

Milan Institute of Cosmetology would like to state that MIC does not request more than the student requires, or what the United States Department of Education allows, and MIC certainly does not hold funds from students.   The student did not supply the necessary documents to complete...

verification for the 2016/2017 funding year until November 29, 2016. It can take a minimum of 90 days to be completed. However, the United States Department of Education per HEA Sec. 484(c); 34 CFR 668.16(e); 34 CFR 668.32(f); 34 CFR 668.34; may reject the student due to the failure to complete the program in the time allotted for the completion of the program as required by satisfactory academic progress.   Since the funding request has not yet been submitted, Ms. Rodriguez needs to submit a letter in writing as soon as possible to her Educational Finance Advisor at MIC requesting a reduction for the appropriate amount needed to cover her balance. This should satisfy the requested desired settlement.

Re: Complaint ID 12314880 – Milan Institute Amarillo, TX   Milan Institute (MI), Amarillo campus, welcomes the opportunity to respond to the complaint, dated August 7, 2017, and filed by [redacted] (“Ms. [redacted]” or the “parent”) on behalf of her daughter [redacted] (“Ms. [redacted] or the “student”) who was enrolled at Milan Institute in Amarillo, TX.   In her complaint, Ms. [redacted] stated that her daughter Ms. [redacted] has yet to receive a refund check for her tuition and that we have mailed five checks, all of which were lost in the mail or mailed to the wrong address.   Ms. [redacted] was due a refund in the amount $1,486.21 from her cancelled enrollment. This was first sent to her on 3/15/17, check #90590. After being contacted by the campus and informed that the check had not been received, our Accounting department at our home office confirmed the mailing address, and then the check request was sent to have it voided and reissued on 6/14/17.   A void and reissue was processed on 7/10/17, and check #91407 was mailed to the address on file, which was P.O. Box 1141, Clarendon, TX 79226, and the postmark was 7/11/17.    On 7/18/17, we received an email from Ms. [redacted] stating that Ms. [redacted] had yet to receive the check. Our Accounting department at our home office called her as requested, and we apologized for the inconvenience. We advised her that the check had been personally sent by one of our accounting analysts at our home office, but it would likely take more than five business days to reach her in Texas. She expressed her concerns that it had been mailed before but not received. Our accounting analyst gave her his extension and told her that if the check had not arrived by Friday, 7/21/17, we could make other delivery arrangements other than sending it to a PO box, such as sending it to the campus to be picked up. She indicated that that would be difficult as Ms. [redacted] had moved over an hour away and did not have transportation. We asked her to give the check a few more days, so we could avoid voiding a check that could show up tomorrow and have her wait another week for a replacement. She agreed, and our accounting analyst again asked her to call him if the check was not received so that other arrangements could be made. Our Accounting department received no further contact from neither Ms. [redacted] nor Ms. [redacted].   On 7/25/17, our home office called Ms. [redacted] to follow up on a voice message she had left in our student concerns voicemail box. We had initially called her on 7/11/17 and left a voice message asking Ms. [redacted] to return our call to discuss her concerns, but she never returned our call. We were able to reach her by phone on 7/25/17, but she said she had already been working to resolve the issue with other staff members and that it was “taken care of.” Neither our student concerns nor our accounting department has received further contact from Ms. [redacted] nor Ms. [redacted] to our knowledge.   Upon review, it appears that the PO box address we had on file for Ms. [redacted] was updated in our database on 8/8/17 to a standard mailing address. On 8/21/17, we mailed check 91669 via USPS certified mail to [redacted]. The USPS tracking number for the check is [redacted]36.   We encourage Ms. [redacted] or Ms. [redacted] to contact our accounting analyst at ###-###-#### if she does not receive the check at the noted mailing address by 8/28/17.

However, our records show no student by the name of [redacted] enrolled at either Milan Institute or Milan Institute of Cosmetology located in San Antonio, TX.   As a result, we are unable to properly respond to this complaint.
