Service date of 7/2/2019, provider Seth J., patient account # *** I called Fort Myers Internal Medicine, LLC and spoke to a lady in billing about this bill and she refused to cooperate with me in resolving this false bill addressed to me. I advised her I was going to report it and she did not care.
