Faulty or way too sensitive mouth swabs
I've had three mouth swabs come back dirty and I hadn't used. If these mouth swabs are so sensitive that I can't be intimate with somebody that has used then this is a problem. I've been called a liar, my character's been decimated, I've lost freedom and time away from my loved ones. The hardest being called a liar. One of their lab doctors lied on me under oath. When is this Injustice going to be resolved. How many more people are going to lose their freedom and their good standing because of these faulty mouth swabs? I'm going to file complaints with every agency possible and will be talking to an attorney. My belief is that Millennium health will be paying for this Injustice soon. If anybody reading this has experienced the same problem text me 971-415-8601. Something's got to be done.

There testing is a joke
I did a saliva test last month and it came back positive fore coke and hydrocodone..lol..I never in my life touched cocaine. Ever..my thing was percocet and I haven't touched that in almost a year..im defently looking for another dr who don't use this lab for testing..if it's that easy to fail one of there test I'm sure my future with this dr who uses them is pretty bleak.

Claim they have no oversight for their lab results
I just came back from my pain doctors office, and was advised that I had failed a drug test, the second time, for THC. I am a 73 year old male, that has never used Marijuana, and there should be no THC in my system. I have been in pain management for several years, and until testing was done by this company, have never had any issues with illegal substances, as I have never taken illegal drugs. I was given the results from both tests, and told to contact Millennum Health at the number on the paperwork, 866-866-0605. I did, and was advised that Millennum Health is a third party contractor and cannot discuss any results with a patient. Any conversation would have to be by the prescribing doctor, who has no vested interest in finding out why tests were incorrect. The doctor is not going to call and discuss with them, they have paperwork saying I failed the test. When asked if there was a state or federal agency that I could contact about the problems, I was advised that there wasn't one, as their results could not be faulty. I did advise them, I would be making a complaint with the Texas Medical Board, and hopefully the TMB could tell me who to contact, or maybe act on the faulty reporting by their labs. A patient should ask where the urine testing is being done by their doctors, and if it is Millennum Health, the patient should run as fast as they can to another lab/doctor.

Positive test for 8 different illegal/unexpected drugs including 4 different opioids. Was taking(prescribed) a benzodiazepine and nothing showed for that. My pain management is in Akron,OH, and they ship urine all the way to San Diego Ca for a simple drug screen? The urine was tested nearly 80 hrs after the sample was given. Asked Millennium how far back the testing reaches and they said "ask your doctor". How is my doctor going to have information about YOUR tests? Look on the internet about these clowns. Scam after scam, bullying and Intimidating employees and it seems to be going on still. I guess a quarter of a billion dollar penalty isn't enough to straighten these guys out. Im digging and not stopping until I find out what the game is now.

Review: I was informed by my healthcare provider that I was not to pay any bills for lab work by Millennium Laboratory. They have received payment from my insurance company and are not to bill for the remaining balance. This is an agreement in which Millennium Laboratory account representative made on behalf of Millennium Laboratory with my healthcare provider. There is a sign up in the office stating not to pay. The employees have written on the yellow copy of the lab orders, not to pay, and even the doctor has told me not to pay. I was also informed to write "Hardship" on the bill & send it back to Millennium Laboratory. The healthcare provider is, [redacted]. The doctor is Dr. [redacted].Desired Settlement: For them to honor the agreement with the Florida Spine Institute & stop trying to collect after they have received money from my insurance company.

Pursuant to our recent conversation and the notice you provided me regarding the above complaint please accept this response as the company’s position with regard to this matter. In accordance with the customer’s statement, she was informed by her healthcare provider that she was not to pay any bills for lab work that was performed on her behalf. Please note that this healthcare provider utilizes services from three different laboratory companies. Please be advised that it is not the company’s policy for patients and physicians who utilize our laboratory services to not pay for same. If in fact there is a sign up in the doctor’s office regarding our company we will immediately seek that the office takes down the sign because it is not accurate or valid. As to the patient’s ability to claim for a financial hardship and seek assistance from our company she can certainly apply under our patient assistance program and we will then determine whether or not she qualifies for this assistance. Simply writing the word “hardship” on a Requisition Form and sending it back to our company is not the process and procedure for our patient assistance program. Please also note that I’ve been in contact with our sales team and they will immediately rectify anything that the physician’s office is doing in violation of our policies, practices and procedures noted above. I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this proposed action would not resolve my complaint. For your reference, details of the offer I reviewed appear below.Yes, the physician's office uses 3 different labs. However, the billing office, the lab technicians, and the physician himself have informed me not to pay any of the lab bills that I receive from any of the labs they are contracted with. The signs in the office state the same. I was told by everyone I spoke with in the physician's office that, "Per the agreement they have with the sales reps from the labs they are contracted with, patients are not to pay any bills after the initial bill is sent to the patient's insurance company. Patients are also to write hardship on the bill & send it back to the lab. The lab will not send any more bills after that. The patient isn't to contact the lab other than to write "hardship" on the first bill. Patients will not be sent to collections nor will they continue to receive more bills. Per the state of Florida, patients who are prescribed narcotics are to be drug tested & labs are to accept what the insurance companies pay for the labs without further financial responsibility to the patient."

The problem here, is the lab is not honoring an agreement that a representative from their company has made with the physician's office. The lab can contact every single person from the lab technician to the ordering physician and can verify that the agreement was made. If an employee for the lab is making agreements with the physician's office, the lab should honor it. Plain & simple. If the employee who has made the agreements isn't authorized to do so, then that's a problem with communication within the lab's company.

We appreciate the information provided by Ms. [redacted]. We do not agree with the physician's office position, if made by them, that laboratory invoices are not to be paid for services provided. A summary of our Billing Policy is stated on our website and within same it mentions our Patient Assistance Program (PAP) should be patient be indigent/"hardship". The physician merely noting "hardship" on the bill is not helpful. If Ms. [redacted] is interested in applying for our PAP, she may certainly contact our offices and filing an application.

Review: My son is scheduled to receive lab work quarterly for his medication compliance per PCP. I do not have an issue with labs being drawn. I am willing to pay for labs. My son had labs drawn in February 2015 and May 2015, which were sent to Millennium Health, LLC for results. I have insurance to pay for labs or a portion of the labs. I received a billing statement from Millennium Health for $1592.85 in July or August 2015. I immediately contacted my insurance company regarding this payment. In February Key Benefit (my insurance paid ) over $200.00 for lab work and over $500.00 for the May 2015 lab work. I should have been billed the remainder. Key Benefit informed my to contact to have Millennium contact them. I contacted Millennium and spoke with a female representative who informed me that Key Benefit had not made payment. I asked the Rep to contact Key Benefit and she informed me that Millennium could not make outgoing phone calls. I gathered all my information plus a printout of the payment that Key Benefit made and mailed it to Millennium Health. Keeping a copy of course. I received a phone call about a week or so later in which another female rep informed me that payment was not received. I got another bill for the $1592.85 in October 2015. This time I contacted Key Benefit in November 2015 and the insurance agent had me on conference call with the rep at Millennium. The insurance payment was made by credit card from Key Benefit and was electronically faxed to Millennium Health. The female rep at that time stated that she would have her supervisor make adjustments. December 09, 2015, I received another bill for $1592.85. I do not believe this full amount is owed for these labs. I have contacted and recontacted this company in which it is obvious something else needs to happen.Desired Settlement: I just want my bill to be properly adjusted since my insurance did make payment.

Review: I [redacted] spoke with an agent of millennium laboratories about the conditions of setting up a payment plan on a debt that I owed. There employee had told me that If I were to send $100 monthly it would go tward my debt, nothing more. A few weeks later I called to inquire to see if they had received the money orders I sent.I sent 3 money orders one for$57. 15, one for $103.79 and one for an even$100. Come to find out they said they only received two of the money orders. The money order I sent $100 is just gone they lost it. I have spoke to them numerous times about this issue and I am somewhat at fault.they did tell me that had I had the receipt for the money order they can find it a lot easier but I do not have it.I know I sent it I do not see why they cannot find it.Desired Settlement: I would like for them to find my money order and if they cannot find it I would like them to credit my account for the money missing.I just don't feel that I should be out $100 on their mistake

Review: I received this billing statement from Millennium Laboratories and immediately my wife and I contacted them asking why the charges for the supposed lab tests were so expensive. They claimed it was because they did not have the correct health insurance entered into their system and would correct any adjustments necessary. We thoroughly investigated the billing statement and quickly realized that there was several errors in the billing codes as well as fraudulent claims for phantom billing (tests that were not/could not have been done). This was for a [redacted] that my wife submitted to her doctor's office. We were informed that we were being charged for the "mandatory" urine drug screen upfront out of pocket which costed us ** dollars yet it appeared on the Millennium Billing statement also and the charge was [redacted] dollars. We asked Millennium why this was, they simply claimed it was ordered and a more in depth testing was required. I looked up the codes that were on the statement and seen that we were being charged for tests NOT performed, like a skin test for [redacted], a [redacted], and [redacted] which requires a 2nd [redacted] 30 minutes after the 1st was given ( This DID NOT happen). All tests were supposedly done from a single [redacted]. We also noticed the other issues such as, double billing, service bundling, repeat billing, up-coding, unnecessary lab tests, and once we confronted my wife's doctor, she said she could not understand why these tests were billed and gave my wife a number to call Millennium Customer Service and instructed my wife call them AGAIN and tell them that we would NOT pay this outrageous amount. Millennium is billing us $[redacted] each month since June 16th, 2014, ($[redacted] total) for a single mandatory [redacted] . We spoke to our health insurance provider we were informed that they had up-coded my wife as a patient that had a severe accident, being treated for pain. This IS ALSO A LIE!Desired Settlement: I would like to see these fraudulent charges dismissed and removed. I firmly believe that I do NOT owe this Millennium Laboratories a single cent due to the fact that I was completely unaware of this service being pushed on us without ANY knowledge whatsoever. I also want to see this fraudulent business closed. Health care costs are way too expensive as it is without businesses like this taking advantage of misinformed patients. The doctor even admitted to NOT requesting this so-called in depth testing, as well as all the phantom tests this business claimed to perform. I don't see how it is legal for a doctor to charge ** dollars in office for a mandatory urine drug screen then allow an off site laboratory also to charge $[redacted] dollars for the same test, Unless this doctor is receiving some kind of kick back. Patients should always have knowledge of what they are being charged for as well as how many times they can and will be charged for the ANY tests. I would like to see some justice!

Review: I Currently and have been going to a Dr.[redacted]/Dr. [redacted] office (Medical One) located in Brigantine,NJ 08203 for 2+ years now routinely once a month. I give a urine during my monthly visit because I'm on a certain medication that requires me to pass a drug screening in order to stay on their program and Mellennium Health apparently is the company who processes the screenings . It is a cash visit and no insurance is involved. The urine screen is included in the $125 dr. Appt. Visit. But Millennium Health continues to send me a bill on a monthly basis. I'm tired of it, and they shouldn't be billing me for this. It has been discussed with my Doctor (Dr.[redacted]) and he says the same, that they shouldn't be billing Any of the patients who are in the program that I (me) am in. I request that something be done about this matter please. It's not fair, and honestly it feels like they're fishing to see if anyone will pay them anything knowing that patients don't have to pay them .Desired Settlement: I wish that Millennium Health stop sending me invoices each month and I'd like to see it stopped for the many others as well. Review: In April 2014, I sought medical treatment from [redacted] and made an appointment regarding some pain I was enduring in my knee. While in the patient room, I was approached by one of the Millenium Health representatives, [redacted], about performing a saliva test, which was explained in detail and further advised there would be no charge for this test. January 2015 I received an invoice for the test after my insurance carrier had reviewed it. Millennium Labs had issued the bill. The doctors assistant, [redacted] also advised that [redacted] had mentioned the test was free and she also had had the test done since it was "free". So I was not the only one given false information.2 weeks ago I called and talked to one of the billing representatives and I thought he was going to mail a form to my attention in order to waive the fee. Instead, I received a financial support form, which I should never have received it since I dont need the financial assistance to pay it, I never should of been charged. The bill is for $800 after they went through my insurance.Desired Settlement: I just want this charge waived and for the charges to my insurance to be reversed.

Review: Millennium Laboratories came to our office and explained what they would be doing that might better our practice. They never informed the office that they would be billing our patients insurance and sent our patients bills for over $2000. They have lied to us from the start and just gave us false information. We have lost several patients cause of this company. I would tell everyone I know to stay away from this company they will rip you off. All they are out for is money and more money.Desired Settlement: We would like all of our patients accounts to be taken care of. None of our patients should be getting any bills from this place. We were told that if someone is getting a bill then they will write it off.
