[redacted]. Mr. [redacted] is a retiree of [redacted]. Milliman administers the retiree health and life insurance benefits on behalf of [redacted]. Below is a summary of events regarding Mr. [redacted]'s daughter being dropped from his retiree health coverage, therefore the basis of his complaint. • On April 9, 2015 a student verification letter was sent to Mr. [redacted]. The letter requested that Mr. [redacted] complete the appropriate sections of the letter verifying his daughter's continued eligibility, sign the letter, include an official affidavit from his daughter's school attesting to her current full time student status and to return these documents to Milliman. • The signed letter and affidavit were to be sent to Milliman by May 15, 2015 or his daughter's health coverage would be terminated effective May 31, 2015. • The return address on the letter was P.O. Box [redacted], Seattle WA 98111-9209 • As of May 26, 2015 we had not received the required signed letter and school affidavit so coverage for Mr. [redacted]'s daughter was terminated effective May 31, 2015. • On June 9, 2015 Mrs. [redacted] contacted us regarding the student verification information they sent in. She stated that it was mailed with their insurance premium to Milliman at PO Box [redacted], Seattle WA 98124-6655. Unfortunately, this PO Box is for insurance payment only and only accessed by our bank. So, we wouldn't have received the documentation that was included with the payment. Mrs. [redacted] asked that we resend the original letter. Another copy of the letter was mailed to her on June 10, 2015. • On June 11 we received a copy of the daughter's school affidavit showing that she was a full time student. We have not received the signed letter. • Upon receipt of the affidavit, the daughter was reinstated with health coverage effective June 1, 2015. • On June 15, Mr. [redacted] was informed that his daughter's coverage had been reinstated effective June 1, 2015.
