Initial Business Response / [redacted] (1000, 5, 2015/12/17) */ Several of our staff memebers have been working with the patient and her insurance provider regarding this complaint The insurance provider was not aware that the clinic stated on the patient's card and our location were connected, they thought his was a clinic out of Rochester, MN It was stated that the insurance provider was going to reprocess the claim again The provider contacted the patient to inform them of the status There were some delays due to the priamry care provider not being listed properly which delayed the claim processing The referrals that were placed by the primary provider to both referral locations were faxed and the insurance forms were also printed and brought to the primary care provider's attention The patient was informed this was done and the staff also informed her that they would call once the paperwork was sent to her insurance company
