*** informed the table that they would not be charged for the saladMs***'s sister did not want a new salad.Upon receiving Ms***'s email, the General Manager responded to Ms*** as detailed in the above complaintThe restaurant manager and the restaurant General Manager did not believe a full refund of the rest of the meal was required at the time. Action our company has taken:We believe that Mitchell's handled the situation appropriatelyMs*** and her sister did not request further compensation at the time of their visit, and they stayed to dine and enjoy the rest of their meal without any further concerns of which we are awareIf Ms***'s sister (the individual who found the ladybug in her salad) wishes to contact us, she may call our corporate office directly at ***-***-***We do not believe further action is required at this time

to respond to the below concernsThe complaint and response are as follows: Customer's Statement of the Problem: "I visited this restaurant on August 19, with a party of Our bill came to over dollarshad a gift card that was applied and with everything the final total was $It was not until I had come home and checked my *** bank account that I realized that the server Melissa, Charged my card an extra $(1;m sure she pocketed as a extra tip) despite receiving 20% cash tipcalled the manager Allyson once the charge was finalized on my bank account several days later and was told a claim would be made with the corporate officeWeeks have passed and no word from the corporate office or local restaurantI am extremely upset and disappointed in the company, especially given the history of fraudulent credit card transactions I would expect the company to take this situation seriouslyI would like the amount to be corrected." The Facts (as we see them): According to our records, Ms*** dined at Mitchell's Fish Market in Woodmere, Ohio on August 19, On August 231d, Ms*** called the restaurant and advised that a $tip was added to her bill that she did not authorizeManagement informed Ms*** that the charge would be refunded within to business daysThe credit was applied on August 29thOn September 9th, Ms*** emailed our Corporate Office and advised that she had not received her refundThe General Manager called Ms*** on September 15thDuring that call, Ms*** checked her account again and realized that the credit came through successfully to her account on August 30th which she had accidentally overlookedAction our company has taken: Ms*** received her requested refund on August 30th, and we believe her issues are resolvedAdditional IT and Operations procedures have been put in place as a result of this matterWe hope that Ms*** continues to enjoy dining at Mitchell's and visits us again soonTo resolve any future questions, Ms*** is welcome to call our Corporate Office Customer Relations team directly at 713-850-
