To Whom It May Concern, The customer indicated was seen in our office initially on October 13th, 2014 for an emergency visit. He is in Erie for school and is originally from California. He contacted us because his crown on...

his front tooth fell off and went down the drain. He wanted it repaired ASAP. Upon the examination it was determined a new crown was needed. The doctor fit him with a provisional (temporary) crown and informed the patient he needed to return in order to complete the permanent crown restoration. The patient indicated he did not want us to fix the tooth he wanted to go back to California and have it done there. He just wanted us to make him something so it looked normal. We explained to him that the provisional crown was not made to last and because of the short prep of the original crown and his bite we could not give him any guarantee on the provisional crown. On October 30th he called stating it had come off. He came back into our office after hours that day and we fit him with another temporary crown which included refining the prep. It was not a quick and easy appointment. We insisted he pay for all the services provided because what he wanted us to do was not meant to fix the problem but just get him by until he could get to California. It is not our opinion that that the first provisional crown failed in an unreasonable time frame for the reasons stated above. We were very clear as to what was needed to fully fix his problem. As you can see from the attached account ledger he was charged $375 for date of service 10/13/2014 and $150 for date of service 10/30/2014. He paid nothing on 10/13/2014 and paid the $150 on 10/30/2014. He then had a balance of the $375 remaining from the 10/13/2014 date. He called into the office and expressed his concerns that the ‘temporary” crown did not last long and disputed the fees. We explained to him again that it was not meant to last forever and agreed to give him a discount of $ 150 toward the remaining balance of $375. That was done on 5/12/2015. His balance then became $225.00 which he paid on 6/16/2015.All of that being said our total charges were $525.00. He paid a total of $375.00. We did a courtesy discount of $ 150.We have contacted the insurance company and they have told us numerous times that we do not have to do a write off for non-covered services. Review: In Oct. 12, 2014, I went into the office to have a temp. crown made. However, it failed on 10/29/2014 and I went in to have it redone. The office was very insistent on charging $150 for the rework even thought the crown had failed in an unreasonable time frame, but I still paid nonetheless. Eventually, the insurance company denied payment for the initial crown. In Januaraym 2015, I had a permenant crown made in California. Later on May 12, 2015, after talking to them, they decided to do a "courtesy discount" of $150 for the second crown. They then agreed to send a new bill to show what I still had to pay. On June 16, 2015 I paid $225 for a total of $375-enough to cover the list price of the initial crown. After some personal analysis and talking with my insurance company, it has been widely determined that I was supposed to pay the contracted price of $251 even though my final crown was not done with them and has since claimed that they are not required to do a write off according to the insurance company.Desired Settlement: I would like a refund of $124.
