To whom it may concern,This patient, [redacted] , was seen in our office on August 4, for medical treatmentIn order to disclose the type of treatment performed we will need a signed HIPAA form.In response to your inquiry, she received one bill in error on 12/7/She contacted our office on 12/11/and it was explained to her that this bill was sent in error based on pending insurance payment and she may disregard as her balance is zero.She has not received a single bill from our office since that point in timeShe contacted our office last week to state she hadWhen we explained to her that no bill has been sent from our office that she should provide us with a copy of the bill to verify where she is receiving this from, she angrily berated our staff and stated "My arm is getting weaker from tossing out bills." We explained to her that the bill she may be receiving can be from a laboratory that is not related to our office and is not within our control.She was again explained that there is no bill from our office and there has not been anyShe has notreceived a single threat and there has been no collections agency contacted as she simply has a Zero balanceShe is unfortunately not willing to provide a copy of the bill in question so it is impossible for us to address as again we have not sent her anything.As for her copay, this is related to her medical services provided on the date of service and cannot be refunded as it is a part of a service provided and is a part of her contractual obligation with her insurance company.We ask that she provide proof of harassment as we have simply not harassed herShe contacted our office once in December and she had no balance to payOur system tracks bills by date of mailing.Please let us know if we can help in any further way but in order to release billing statements and medical records we will need a signed HIPAA release form.Erum l MD
