Dear Sirs,Please accept this communication as our response to your February 1, 2016 correspondence regarding Ms. ***’s request to cancel her annuity policy. After a complete review of her annuity policy file, we acknowledge our errors and would like to provide two options to resolve this matter.We... strive to offer the best possible service to all our policy owners; however, we have failed to meet our own standards on your annuity policy. We agree we have made errors on two of the three withdrawals we have processed from your annuity policy.To show our sincere apologies, in the hopes to keep Ms. [redacted] as a valued policy owner, we would like to give her $1,000.00 to cover her taxes. This amount would be deposited directly into her bank account using the information she has previously provided. This goodwill gesture will not be taken from her annuity policy and will not be reported as income to her.If the above offer is not acceptable, we will allow Ms. [redacted] to surrender her annuity policy with no withdrawal charges. The premiums can be deposited directly to her bank account or we will transfer the premiums to another financial institution with required documents. We would like to remind her, if the premiums are remitted to her, we will report the transaction as income on a 2016 Form 1099R in January 2017. If Ms. [redacted] would like to have premiums transferred, we will require transfer documents with a letter of acceptance from the financial institution. According to IRS regulations, trustee-to-trustee transfers are not a reportable event.We hope we have adequately addressed all your concerns. I also tried to reach out by phone, however the phone number provided goes directly to voicemail. I received a call from *** who offered no relief* did not want to respond on the Revdex.com website citing privacy , but once I filed the complaint I gave the Revdex.com permission to be involvedLike I said I was mislead I just had major surgery to my ankle which my doctor said I may not walk the sameMy surgery went from a repair to a reconstructionWhich is causing me to miss work minimum the next weeks and I was told by a representative I had to be dying to receive benefitsI was told I would receive a letter to cancel the policy which I never receivedI was told before anything I would receive a letter to fill out to file a claim I never received thatThis company told me a lot of things that proved to be then tried to place blame on the agent in which I got information from the company contacted the agent she contacted the company and was told the same information told to meI would receive a questionnaire to file a claim I never got itThe company told me they sent a letter to my PCP about my surgery when the PCP is not the surgeonThe company did not meet my needs and now after speaking to *** I feel even more I was scammed*** was unempathetic to my situationI was called the day after my surgery still kind of sedated and in pain.
