*** *** received medical services from Valley Physician Services on dates of services 10/14/& 12/9/Per her insurance carrier (***), Ms*** was deemed responsible for a deductible of $for date of service 10/14/& a co-insurance amount of $for
date of service 12/9/Ms*** paid $towards the above balances, leaving a remaining balance of $Ms*** was sent statements reflecting the amounts due on 10/26/16, 11/30/16, 1/4/17, 2/8/17, 3/15/17, 4/19/17, 5/24/& 6/28/In addition, she was offered a payment plan to assist herPayment installments were not received towards these balances which resulted in her balance of $being transferred to collection initially on 8/6/Our customer service staff attempted to explain the above mentioned balances to the patient on several occasions & provided her with the details reflected on the explanation of benefits forms her insurance carrier provided and this was not satisfactory to herThe $balance reflected in Ms***’s account is validCopies of the patients insurance carrier (***) explanation of benefit forms for the applicable dates of services & the statements sent to her can be provided upon requestSincerely, Kathy K*** Manager, Billing Valley Physician Services, PC

I am rejecting this response because: I've paid over $100 from my HSA which I have sent them copies of. I sent them to the collection agency in addition to the emails back and forth between me and Time who confirmed first the balance of $41 then the balance of the $386. I've been paying since April. I don't know where THEY apply the payment but I've been paying nonetheless. I've sent the collection agency 12 pages of proof and the billing agency 12 pages Twice. Please provide an address I can send you the 12 pages of proof of payment and communication between the rep and I. Being that I pay $25 a month for the past 8 months, there is no reason I should be put in collections. I'm paying. It's not my problem if they don't put the money towards the outstanding claims first. 
Please let me know your address that I can send all the pages to.
