Thank you for giving me the chance to respond to the correspondence you received from our member, [redacted]Ms. [redacted] did open her account as stated on September 29, 2017 at which time she was informed the $25.00 is required to become a member and open the shares/savings...

account. The minimum $25.00 must remain in the account six months to avoid the early closure fee. At the time ofopening her account, a debit card was processed and ordered for Ms. [redacted]. Unfortunately, the card did not arrive to Ms. [redacted] as stated. She came into the office on October 23, 2017 and a new instant issue card was issued to her at that time. OFCU began offering a new feature, instant issue cards in October, allowing Ms. [redacted] to receive a card immediately when shearrived the second time. The new card was active and Ms. [redacted] began using the debit card on October 24, 2017 with no issues.                                               OFCU at some point receive return mail and the account was flagged with a bad address, which caused the $2.00 fee to be assessed on October 31'. OFCU would gladly refund that fee when members come in and provide the correct mailing address. The fee was refunded to Ms. [redacted] on November 6, 2017.Ms. [redacted] was contacted by phone on November 6, 2017, by our SVP of Operations, Elsa M[redacted], apologizing for the delay with the initial debit card and any inconvenience she encountered. Ms. [redacted] was asked if there was anything we could do to maintain our relationship, and offered to waive the account closing fee if she chose to continue with closing the account. OFCU made every attempt to rectify the situation for Ms. [redacted].
