[To assist us in bringing this matter to a close, you must give us a reason why you are rejecting the responseIf no reason is received your complaint will be closed as Answered] Complaint: [redacted] I am rejecting this response because:I do not agree with this statementOn every occasion that I received a bill I had contacted the hospital and I was assured that the bill was settled and that I had a zero balance due Furthermore, I have received a letter from the hospital stating that I have no unpaid balancesLastly I have called the number provided on the bills received on the dates you mentioned and no one ever answeredThis all appears to me as a fraudulent business, it is very suspicious activity and might be related to identity theft since they require me to add my social security number as an identifyingI have personally contacted the physician that cared for me (Dr.David S [redacted] ) and he assured me that this type of request for payment do not seem legitimate and was advised to check it with the Revdex.com for additional securityI have received a letter from the hospital that all my bills are payed, and have received confirmation from my physician as wellEven though I received confirmation both verbal and written by hospital and physician these fraudulent looking bills still come and harass me on pass due chargesI wish to get this resolved as soon as possible, so that no one has to experience any fraudulent acts and risk getting their social security number and other important information stolen and used against them Regards, [redacted]

Mount Lassen Emergency Physicians has mailed a statement of account as a result of this emergency room visit in the amount of $to ***A claim in the amount of $was submitted to the insurance carrier on file- *** *** *** Health Plan- but was denied as they were unable to identify the patient as being insuredI understand from reading the complaint that *** believes this bill has been take care of and is not valid Our office does the billing for the emergency room physicians. This bill is for the direct treatment and/or supervision provided in the Emergency Department by a healthcare professional. Mount Lassen Emergency Physicians consist of a group of doctors who work out of *** Medical CenterThese doctors are independent contractors and are not employed by the hospital; therefore the Physicians must bill separately from the hospital. Unfortunately patients will receive a billing statement directly from the facility as well as a billing statement from the physician. Any payments made directly to the hospital will not be included in our bill Our records indicate that we mailed two statements to *** on 7/22/and 8/23/Unfortunately, his account is showing past due and delinquent due to not receiving a call or correspondence regarding the accountIf he has any questions about his insurance taking care of the bill, I would direct him to contact *** to verify coverage and benefits The original medical record shows that *** agreed to an evaluation and treatment by DrS*** in the emergency department*** may also contact the facility to obtain a copy of his medical record If *** believes there is a billing error or a registration error, I encourage him to contact our office at ###-###-####, option#*The office is open AM - PM EST If you have any questions or concerns, please feel free to contact us directly Thank you for bringing this matter to our attention Very Truly Yours, Danielle H*** Patient Service Supervisor
