The verifier stated "*** does charge a monthly service fee of $per month for each line and a flat rate of $0.05cpm for all interstate calls." The verifier informed Mr*** that *** was not affiliated with the local telephone companyMr*** provided his month and year of birth of *** *** as proof of his identity, authorization, and intent to switch to ***.After completing the verification, the verifier telephoned Mr*** at *** to reconfirm the sale and to reiterate that *** was unaffiliated with the local telephone companyAttached is a copy of the recorded third-party verification file.The above account utilized 1,minutes of *** long distance services through October 29, 2015.The account was canceled, blocked from incurring further charges, and added to *** internal do-not-call list on November 4, at the customer's verbal request.As a matter of professional courtesy, *** has issued the customer's requested credit (in the Revdex.com complaint) of $to the accountPlease find attached the Revdex.com complaint for your referenceThe credit will be reflected on customer's local billing statement within the next one to two billing cycles.Very truly yours, Alicia ** ***Regulatory and Compliance ManagerEnc: Third-Party Verification Revdex.com Complaint

*** *** As of yesterday mail, I have not received a copy of the letter belowThe only e-mail I have received was on 2-10-that I was to ignore the billing.MILDI is not being honest with you I have a copy of the cancelled check and notice of cancellation they received in Dec They also were sent a letter in Janwith a copy of that check and letterI have a copy of all that correspondence alsoSO, they WERE notified in writing TWO times before FebSo for them to claim "ASIDE FROM THIS SINGLE COMMUNICATION, MLDI HAS NO RECORD OF ANY PERSON CONTACTING MLDI FOR ANY REASON" I is false.I repeat this is false They have only come clean after I contacted the Revdex.com I feel this is a scam and should be treated as one.I know I am not the only one that this has happened to Check on line for other complaints about this company.Thank you for your assistance in this matter and keep up the good work!

I have reviewed the response made by the business MLD in reference to complaint ID [redacted], and have determined that this only partially resolves my complaint.
The attached letter sent to the Revdex.com by MLD was formal, informative and clearly explained their bill. It would have been appropriate for the customer to have received such a letter with the first bill, at the correct billing address. Instead MLD sent their first bill to the service address with NO EXPLANATION. Prior to this, phone bills were from [redacted] ONLY and sent to a separate billing address.
Their bill was quite generic and looked informal. My mother does not typically receive bills at her home and I had never heard of MLD. My mother, [redacted], has dementia and has been victimized before. It seemed like a scam.
I spoke to and exchanged messages with MLD several times. The individuals were courteous and informative. I felt comfortable this was not a scam and I resolved to make payment. I then received a message from MLD as[redacted] if I wanted the service terminated. I returned the call and left a voice message as[redacted] that the phone service NOT be terminated, and that payment was forthcoming.
Without notice or contact from MLD, my mother's outgoing phone service has been terminated. My elderly mother now cannot call me or anyone from her phone.
My complaint is now amended as follows:
MLD did not send their bill to the correct billing address.
MLD did not adequately inform the customer/payer of the change in billing.
MLD inappropriately cut off service with no notice or information on how I am to reconnect service.
I have no idea if we have a choice in "long distance" companies to change service to, or how I am to reconnect with MLD if possible. It appears my number is blocked with MLD so I am at a loss right now.
I very much appreciate Revdex.com's assistance in this matter and would like to resolve it quickly.

spoke with Mr. [redacted] who identified himself as the Vice President of the business. The telemarketer transferred the call to an independent third-party verification company to be verified as a sale in the ordinary Course of business.Mr. [redacted] confirmed with the verification operator that he was over the age of 18 years old and authorized to make changes to the business telephone account. The verifier specifically asked Mr. [redacted] if he was authorizing MLDI to become the new long distance provider, and he responded "yes." The verifier informed Mr. [redacted] that MLDI was not affiliated with the local phone company. Mr. [redacted] provided his month and year of birth of December 1957 as proof of his identity, and his authorization and intent to SWitch MLD.After completing the verification, the verifier telephoned Mr. [redacted] at [redacted] to reconfirm the Sale and to reiterate that MLD Was not affiliated With the local phone company. The verifier informed Mr. [redacted] that the new long distance service with MLD would begin in approximately 5-10 days. I have attached a copy of the verification in the form of a (wav file) for your review.On December 14, 2016, MLDI canceled the above account upon the receipt of Mr. [redacted]'s complaint. In addition, MLD has canceled the charges that were billed on the December 7, 2016 invoice of $29.79, bringing the account to a zero balance. Finally, MLD has placed the above telephone number (and the additional two numbers affiliated with the account) on its internal Do-Not-Call list.Based upon the foregoing, MLDI believes that it has conducted itself in accordance with FCC and Commission regulations in all regards in this matter. The letter does not address any of the issues.ACCOUNT: [redacted]Removal of the charges has not resolved the problem. Payment has been sent. MLD did not IDENTIFY themselves and effectively communicate, the change in billing until Revdex.com got involved. Now that it is clear that MLD is the legitimate long distance carrier, my elderly mother has no outgoing phone service.I will again outline my amended complaint:MLD DID NOT COMMUNICATE CHANGE IN BILLING OR IDENTIFY THEMSELVES TO THE PAYOR, RESULTING IN ORIGINAL Revdex.com COMPLAINT.MLD DID NOT EFFECTIVELY COMMUNICATE SHUT OFF IN SERVICE.I CALLED MLD AND REQUESTED THAT SERVICE NOT BE SHUT OFF AND ADVISED THAT PAYMENT WAS BEING SENT. MLD SHUT OFF SERVICE WITH NO INFORMATION ON HOW TO RESUME SERVICE.MLD DID NOT RETURN MY CALL TO RESUME SERVICE.I have no information on how to resume service or if there are any other carriers that provide the service. Any assistance from the Revdex.com to resolve this problem would be greatly appreciated.Regards,

an independent long-distance reseller. Until December 31, 2017, MLDI billed many of its customers via [redacted]'s local phone bill. Effective December 31, 2017, [redacted] discontinued its relationship with any independent resellers billing customers through their local phone bills. MLDI notified its customers of this change in billing format via a notification letter sent in November 2017. As a courtesy, MLDI has been cancelling and crediting balances for any customers that do not wish to transfer to a direct billing format.MLDI canceled the above account on February 16, 2018 and blocked it from incurring any additional charges. The delay in cancellation was due to the fact that Ms. [redacted] did not provide her mother’s name, phone number or account number in the complaint. We reached out to Ms. [redacted] via email to obtain the account information.As a matter of professional courtesy and while admitting no wrongdoing, MLDI removed the invoiced charges of $40.48 bringing the account to a zero balance. Additionally, the billing telephone number referenced above has been added to MLDI’s internal Do-Not-Call list. Please note, MLDI’s long distance services were utilized. In fact prior to cancellation, 473 calls were placed using 954 minutes of long distance service.Based upon the foregoing, MLDI believes that it has conducted itself in accordance with FCC and Commission regulations in all regards in this matter. Until recently, MLDI utilized [redacted] Billing Services Group as a billing aggregator that enabled MLDI to bill for its services on a consumer’s local telephone bill. In November 2016, MLDI began directly billing its [redacted] customers for long distance services. Any customers affected by this change were notified prior to this transition via physical mail.MLDI cancelled the above account on February 10, 2017 (upon the receipt of Mr. [redacted]’s email) and blocked it from incurring any additional charges. Aside from this single communication, MLDI has no record of any person contacting MLDI for any reason. Nonetheless, as a matter of professional courtesy and while admitting no wrongdoing, MLDI removed the invoiced charges of $22.63 ($12.02 billed on December 28, 2016 and $10.61 billed on January 30, 2017) bringing the account to a zero balance. Finally, the billing telephone number referenced above has been added to MLDI’s internal Do-Not-Call list.Based upon the foregoing, MLDI believes that it has conducted itself in accordance with FCC and Commission regulations in all regards in this matter.
