I have reviewed the response made by the business in reference to complaint ID ***, and have determined that this proposed action would not resolve my complaint. For your reference, details of the offer I reviewed appear below.I was not provided the documentation recently requested by Ms*** as per all signed treatment plansThe Dentist never stated to me that she would need to see or possibly see an endodontistMy oldest daughter was at every appointment with my daughter and myself she can verify that no one ever called us back to talk about treatments or sign papersI am requesting that some form of compensatation towards the trouble of the dental work that needs to be done

Hello, This patient was seen in our office four times for treatment, (2013, Feb 2015, March 2015, & June 2016). On 02/25/we advised the parent that the patient had deep fillings and would possibly need to see an Endodontist. The parent received verbal and printed
information regarding the treatment her child needed. She signed the form that explained the treatment and the cost. She was only liable for her $deductible. We did not charge her anything else. On 03/18/the patients treatment was completed. Our doctor personally spoke with mom and explained what treatment was done. All items are documented and filed in the patient chart. On 06/16/they came back to our office months after the first treatment date because the patient claimed to still be having pain. We took x-rays and adjusted the filling. The x-rays clearly show that we completed the three fillings from March. After adjusting the filling the patient told us it felt better. We advised them to call us if she continued to have pain. We did not charge them for this visit. We haven't heard from this parent until 8/3/(months from last treatment date).I personally reached out to mom once I was made aware of her complaint.
