Our records indicate that on September 1, 2009, Mr*** enrolled in a membership in the Privacy Matters Identity program when he completed an order form via the Internet at a website sponsored by neverblue.comMr*** also received a membership in Privacy Matter when signing up for the Privacy Matters Identity program
The landing page through which Mr*** enrolled explained the terms of the offer, including, that the programs came with a seven-day trial period, during which he could use the benefits and he would not be charged the monthly membership fee until after the trial periodIt further explained that during the trial period, Mr*** could cancel the programs without incurring any cost by simply calling our toll-free numberAdditionally, it was explained that the monthly membership fee for the programs would be automatically charged each month to Mr***'s accountThe landing page also stated that Mr*** could cancel his memberships at any time after the expiration of the trial period and would no longer be charged the monthly feeThe landing page also explained that the membership fees would be charged each month automatically to ensure continuous service, unless Mr*** called to cancel the programs
In order to enroll in the programs, Mr*** was required to complete an online order form and provide his personal information, including, his name, address, and credit card numberBased on his acceptance of the offers, the program information was sent to Mr*** confirming his enrollment in the Privacy Matters Identity and Privacy Matters programsIn addition, the information also provided him with his membership ID numbers and as well as links to the Privacy Matters Identity and Privacy Matters programs sites so Mr*** could access his benefits
On September 8, 2009, after the trial period ended, Mr*** was charged the $monthly membership fee for the Privacy Matters Identity programMr*** was also charged the following: 1) monthly fees of $29.95; 2) monthly fees of $34.95; 3) monthly fees of $39.95; 4) monthly fees of $44.95; 5) and monthly fees of $when the membership was in effectOn July 31, 2015, Mr***'s Privacy Matters Identity membership was cancelledAlthough Mr*** did enroll in the program, customer satisfaction is important to usAccordingly, we issued a full refund of all Privacy Matters Identity membership fees in the amount of $2,
