There is no reason why Shady Grove should refuse to reveal to me how they decided what to charge me (or anyone)Arbitrarily making up costs is a terrible business practice and unfair to patients? As a follow up, physician had a lengthy phone conversation with patient today - patient has made a Follow up appointment Supervisor is calling patient this afternoon/tomorrow morning to assist with scheduling that appointment

Patient paid according to a global fee schedule In the deposit sheet she received and agreed to - it states: "Due to the nature of this program, itemization of cost for insurance reimbursement is not possible Itemization for the sole purpose of applying charges to a deductible or
out of pocket maximum is also not possible To participate in this discount global fee program, you acknowledge that you are waiving your insurance benefits and claims will not be submitted to the health insurance company for services included in the program Documentation for payment will be issued in the form of a receipt".Patient could have had insurance coverage for the treatment if she waited for open enrollment - she declined, opting to participate in the above global fee discount program.Patient's initial charge of $10,is correct Mid-cycle, she decided to do pgs testing and in April paid the additional $5, The refund portion of not going to FET is a flat fee of $which was requested to be refunded in May and is in the works Checks take approx 4-weeks to reach the patient There is no documentation to support she was told she would get $or $back - we sincerely apologize if there was miscommunication in that regard

This check was issued initially in a timely manner post treatmentWe have requested a stop pay and stat reissue to our patient. *** *** is correct, she is still entitled to a refund which is now in processI have requested this to be done ASAP. We will be sending detailed breakdown of charges for services we have provided and payments we have received from *** *** to dateWe are responding to her directly via certified mail. I am rejecting this response because: the response does not address the complaint The letter states at the embryos belong to both of us, which may be true It does nothing t acknowledge that all bills have been addressed exclusively to me They have not billed the father and do not require that he contribute to the cost, and the letter does not deny that this is the case It would be simple to acknowledge and remedy the situation

Fertility mails out embryo disposition forms on a regular basis providing
notification to the patient that they may complete and return an embryo
disposition form to either transfer, dispose or donate to research their
embryos that are in storage or they will be
billed for the storage charge. According to our records, Nelson G***,
the Shady Grove Fertility staff member who manages these accounts, has been
reaching out to both *** *** and her partner regularlyMrG*** has
provided both, individually, embryo disposition forms and has spoken to both
attempting to have them come to some agreement to; complete the necessary forms
and ultimately move to a resolution on donation, disposal, or storage payment. Years later and we have not received any
disposition forms from *** *** or her partner. The decision to freeze the embryos was made
by both parties in when they both signed the necessary consent forms. SGFC performed the requested services and

I am writing in response to the complaint that your office received from [redacted] became our patient on March 6, 2015. During her time as our patient moving through the pre-screening process toward treatment there have been numerous occasions of disagreement between [redacted]...

[redacted] and our clinical team regarding how to proceed and Shady Grove Fertility policies.On May 19, 2015, [redacted] spoke with our Clinical Operations Director regarding her frustration with the process. [redacted] felt the requested consult by her physician, Dr. S[redacted] and his decision to cancel/delay her fiancés upcoming procedure until the consult took place to be unnecessary.On May 20, 2015, Dr. S[redacted] spoke with [redacted] and reviewed the current treatment plan and progress and discussed her concerns. [redacted] stated she had lost trust in Dr. S[redacted] and the clinical team regarding her treatment. We place a high value on the patient physician relationship and this statement coupled with the struggles that have been experienced getting to this point in treatment made it apparent that [redacted] would be better served by another practice. Dr. S[redacted] made [redacted] aware of this fact but stated he would seek counsel from the practice's administrative team and get back to her the following day.On May 21, 2015, I, Vicki G[redacted], contacted [redacted] to reaffirm the decision to dismiss her from the practice and assist her in obtaining her medical records. While she was sad to hear the final outcome, she stated understanding. Today, May 22, 2015, [redacted] submitted a request for Medical Records, which were able to quickly respond to as she stated she had a new appointment elsewhere on Tuesday 5/26/15.If you have additional questions, please do not hesitate to contact me.Sincerely,Vicki L.
