Dear [redacted] , Thank you for voicing your concerns, I appreciate your feedback I am truly sorry that our laser hair reduction treatments have not been as effective as you would like As I looked into your statements and your situation, here are some of my thoughts: We are the largest training facility in the country We train both medical and non medical professionals to be certified laser technicians We will always practice, teach and promote safety with lasers This means: We ALWAYS look at the patient we are about to treat to understand their history and skin type Based on the above, we will make a conservative choice on how to treat the client We do not guarantee that our laser hair reduction treatments will result in permanent hair removal There will always be some touch ups necessary Some people will need more than others We do promote that you schedule your treatments based on the hair growth cycle This will produce the most success When you communicated with the manager of the spa in March of 2013, you told her you were unhappy with your laser hair reduction results She came to me to help her find a solution We offered you your choice of: unlimited hair reduction in our training facility OR free treatments in a private setting It is my understanding you opted for the unlimited treatments in the training facility You then came back to the manager in April of to express your displeasure with your treatments on your face and underarms According the notes in your account, she gladly replaced the treatments you had on the original machine with complimentary treatments on the other machine Let me state for the record, that the machine you were originally put on is perfectly safe and acceptable to be used with your hair and skin tone We leave treatment specifics up to the expert in the room and [redacted] and [redacted] decision to work on that machine vs any other machine was 100% acceptable According to my calculations, we have given you over $2,of complimentary treatments Whether it was in the training facility or privately, National Laser Institute has replaced each and every treatment you were unhappy with We wish you nothing but the best, and again, thank you very much for your feedback Sincerely, [redacted] Owner/founder National Laser Institute

With all the respect to the company, I personally think that no matter how large a business is, what matters the most is the client’s (customer’s) satisfaction and the way the company treats people
a The first day (orientation day) they did not check my skin/hair type (didn’t even look at it)
b What they guaranteed to me was that: you will not have any hair growth for the rest of your life
c If you would look at the system you will see the irregular treatment appointments days (due to the reason that I always picked my appointments days)
When I called/e-mailed/talked to the manager (for multiple times) on due to my dissatisfaction, she did not responded me at all, and that made me to find your (owner’s) e-mail address and contact you directlyAnd you did gave me an option of eight free treatments (due to the reason that I had passed and paid for eight treatments) with an instructor (I’m wondering why you mentioned three free treatments in your e-mail) or unlimited treatments with students
And I went back to the manager due to the dissatisfaction of face/underarm treatments and what she exactly told me was: “I will give you free treatments for face/underarm with students, but my owner doesn’t know anything about it, if he knows he will be mad” (which shows the communication between the manager and the owner)
I still did not get any reasonable answer to the question that why they used “light sheer” machine which is for light skin/hair type for me who has thick hair in the situation that they could use a better one?
I still want my money back which is 2355$ because no matter how many free treatments I had, I’m still not satisfied and didn’t get the resultAnd even when I subtract the free treatments I still don’t understand the way the owner calculated that (I’m wondering how the owner came up with over 2000$ after his calculation?), I would like to see the calculation due to the reason that all my free treatments were with students and student treatments are half a priceSo, I paid 2355$ and half of this price is 1177.5$ which means the company still owe me 1177.5$

National Laser Institute provides safe and effective treatments to thousands of clients each year.  When our clients follow their treatment protocols, they almost always have great results.  Despite our best efforts, this customer did not follow protocols after she was treated. We have documented records of our extensive follow up with this customer to address her concerns and find her comments to be blatantly false. We pride ourselves on our communication and responsiveness with all our clients. Once this client involved her attorney in the matter, the communication and follow up has been through our legal advisers to her attorney.  As a note to our clients, proper post laser treatment protocol calls for limiting exposure to heat (sun exposure, jacuzzi, extensive exercise or products that aggravate the skin) to avoid issues. [redacted], CEO

Ms. [redacted] attended National Laser Institute in January of 2013.  She contacted us when she was told by her employer that they were not able to insure her for doing cosmetic injections.  In our profession it is...

the responsibility of the licensed professional to work within the scope of practice that is regulated by their board in their particular state where they work.  We train professionals from around the country.  We do, however, try to stay as up to date as possible on all the laws and rules. 
 

Upon researching the situation that Ms. [redacted] brought to our attention, National Laser discovered that the Arizona Nursing Board had released an opinion that excluded LPNs from being able to inject boulinum toxin type A, sclerotherapy and dermal fillers.  Her instructors and admissions representatives at National Laser did not have any knowledge of this change in "opinion" issued by the Nursing Board.  The opinion is not a rule or a law, but may have been the reason that the insurance company was denying coverage.

 

Even though we were under no obligation to do so, National Laser Institute's COO refunded the entire amount of tuition that Ms. [redacted] had paid to learn to deliver these injections in the amount of $4,600.  At the time Ms. [redacted] remarked that she enjoyed and valued the training, and was happy to recieve the refund.  Please see attached email string to verify our conversation.  Ms. [redacted] was able to still continue to practice cosmetic laser procedures on which she also received training.

 

Since National Laser had been paid tuition directly from a lender, the refund was paid back to that lender.  National Laser was forced to absorb the cost of instructors, model clients, supplies and facilities for the training that we refunded, but we graciously refunded that portion of the tuition to remedy the situation.

 

National Laser Institute has a graduate services program that sends out job leads in the industry to individuals seeking employment.  While our Graduate Services representative cannot offer a job to anyone, her role is to send job leads to our graduates who are interested in receiving them.  It is up to each individual to follow up on those job leads.  Our Graduate Services representative would be happy to continue to help Ms. Ostoich, but to our knowledge has not received communication from her in at least 4 months.

 

While we are very sympathetic to the fact that Ms. [redacted] had not yet found a position she desires, we feel that refunding her money for the training and offering to send her job leads is a fair resolution and goes above and beyond what a reputable education institution would do in this situation.

Review: The last week in May of 2013, I went to the National Laser Institute for laser treatment of sun damage/freckles on my arms, legs, and back. I spoke with my technician, [redacted], about my concerns regarding my skin as it is olive on color. She assured me that the treatment would be safe. My arms were severely burned from shoulder to wrist. Everywhere on my arms that the laser touched, was burned and now lacks pigment. One sunspot that was more pigmented than the others was lasered repeatedly. This spot was lasered raw, the skin burned away, exposing red flesh. I remember seeing it sizzle when it was hit with the laser an additional time. The sore that was made was the diameter and depth of a cigarette burn. The institute had been treating my arms with some type of oxygen application once a month. I am not sure what that does exactly. As a result of the burns, my arms have lost the pigment everywhere the laser touched my skin. The scarring has taken the form of white blotches and left depressions in my skin. The technician, [redacted], told me the scars are there permanently and to stay out of the sun forever.At my last visit for the oxygen treatments on my arms, I pointed out a brown spot on my face. [redacted] offered to "hit it" with the laser that was in the treatment room. It was lasered three times. A blister formed and when the scab came off, there was a ring of burned skin and a deep hole in my face surrounded by scarring. The sore was not healing and two weeks after the treatment, at their request, I saw a Dr. [redacted] who works(prescribing hormones) in conjunction with the Laser Institute. She told me to keep ointment and a bandage on the wound. Dr. [redacted] also said to let her know how I was doing in five days. [redacted], the director, was told to send in my bills for reimbursement which I did. My costs for treatment, make-up, and doctor visits were never reimbursed. These costs, to date, are in excess of $2000. I have pictures.Desired Settlement: I would like the costs for my make-up, [redacted], concealers, creams to cover the burn on my face for an entire year, esthetician visits to minimize the permanent scars on my arms, and doctor visits to be reimbursed. The treatment of the scars on my arms is $100.00 per visit which is once a month and will go on forever. They have offered nothing and were insincere in the offer to compensate me. I was told repeatedly that [redacted] was out of town.

Furthermore, what was written was a fabrication. The statement that the laser school has been in contact with me is absolutely not true. They are hiding behind [redacted] as they have do documentation of any true settlement. There has been NO COMMUNICATION from the manager or anyone from the school since May,2014.They did not make good on their offer to pay my bills and did not ever contact me to see how or if my face/arms had healed! It has been one untruth after another from the school. I have copies of my bills as well as emails from the school. My last email to Dr. [redacted] was not even answered.Please let me know if you were able to open the pictures that are attached to this email. I am a teacher and if I made promised to customers and did not fulfill them, I would certainly be help to task. There is nothing that they can produce in the way of documentation to show Revdex.com to substantiate that they helped me in any way since I was severely burned at the school. Their behavior is disconcerting on many levels. As a business that is supposed to improve people’s beauty, they have failed as they have scarred me for life. Morally and ethically speaking, their behavior is reprehensible as they have been deceptive and untruthful both to me and to your agency as well. Last week, [redacted], CFO, called my number by accident thinking he was calling Revdex.com. I told him that he had reached me, not the Revdex.com and we spoke briefly. [redacted] told me that being notified by the Revdex.com was first time he had ever heard of my problems. I said that my burns occurred in 2013and 2014. He gave me his word he would talk to the people in Scottsdale and call me back no later that the following Friday, March 13th. He never called me back…. and he sounded so sincere too! This failure to do what is promised seems to be pervasive throughout the organization.

Despite our best efforts, this customer did not follow protocols after she was treated. Once this client involved her attorney in the matter, the communication and follow up has been through our legal advisers to her attorney. As a note to our clients, proper post laser treatment protocol calls for limiting exposure to heat (sun exposure, jacuzzi, extensive exercise or products that aggravate the skin) to avoid issues. Review: I attended NLI in Jan 2013 for the CLST, and botox /filler course that consisted of 10 days total. As a LPN (nurse) AZ was a state that allowed there LPNs to do injections, which is the main reason why I relocated from MT to AZ ( to receive this new skill). MY school recruiter was very adamant about me enrolling in the January class, when I was planning on starting a little later. I received my first job working as a cosmetic injector nurse in March of 2013 at [redacted] Healthcare, but that did not last long (ended in June 2013). The Medical Director (my boss ) was unable to attain malpractice insurance for me , he stated that the insurance companies could not give him a valid reason to why they could not insure me. AZ nursing board could not even give him a straight answer. Why would a school advertise this if they knew this was in the grey. I have yet to receive any straight forward answer and feel that I was completely lied to by my school. I spoke with the owner (schools) brother who stated they would pay half of my student loan in regards to the nurse injector situation (was not happy with that) and did not want that, they paid half of my loan anyway. The school also sated that they would put my other skills attained there to good use (CLST) so they directed me to one of there staff members to help me look for employment and I had spoke with this person but one time thus had called in attempt for follow ups with no response. I feel like that one helpful attempt from them was to get me off their back. I feel like I was lied to and it was false representation on their behalf. I have had a financial hardship since then , and would like some resolve on this matter.Desired Settlement: l[redacted] refunded back

Ms. [redacted] attended National Laser Institute in January of 2013. She contacted us when she was told by her employer that they were not able to insure her for doing cosmetic injections. In our profession it is the responsibility of the licensed professional to work within the scope of practice that is regulated by their board in their particular state where they work. We train professionals from around the country. We do, however, try to stay as up to date as possible on all the laws and rules.

Review: I went in to have one procedure done and the technician recommended another procedure, saying it would help with the wrinkles under my eyes and cause minimal swelling. She assured me it would greatly improve my problem. Instead, my face swelled up (about 30 percent) for four days, became red and oozing. Additionally, I called 6-7 times to say I had a fever and my face was in such severe pain I could not sleep. Since we are new to the area I had difficulty getting in to a dermatologist, who I saw on Thursday after the Monday procedure. She said my micro-laser peel was a medium level, serious, peel that would require up to 10 days of down time. The technician knew I had meetings throughout the week I had to attend and said my face would be red and dry. I was not able to get out of bed for at least three days. My skin looks worse under my eyes (more wrinkles) and much of my fat grafting that I told her about (9 months prior it was performed) is gone. I am thinking of filing suit.Desired Settlement: I would like a full refund of my $320, however it will in no way cover the costs of having additional fat grafting done in a year along with numerous Fraxel treatments to repair the damage. I am currently job searching, so this puts me in an embarrassing and uncomfortable position. This technician gave me inaccurate information, overtreated my thin skin and caused deeper wrinkles to form. Additionally, she needed to have sought more advice before doing such an aggressive treatment on my skin, especially since I told her that I had to be up and out thoughout the week. I want my money back and I would like to move on. Otherwise, I will file charges since this has cost me thousands of dollars.

I started my laser hair removal treatments with this company around two years ago. I spent 2355$ for Face/Underarm/full legs treatments (by Lightsheer machine) and I absolutely did not get any results yet. [redacted] (technician at this company) was the one who introduce the process to me at the orientation day and did almost six months of my treatments. On the day of the orientation, she did not check my hair/skin type and she told me that after eight treatments I will not have any hair at all for the rest of my life. After [redacted], [redacted] continued my treatments and she told me the exact same thing, she also added that she had the treatments herself and she is not having any more hair at all (she encouraged me to continue). After eight treatments, I noticed the same amount of hair growth. I communicated with [redacted] and she told me I might need eleven or twelve treatments to get the results (I was not informed before). I tried to talk to the manager ([redacted]), but she was pretty disrespectful and rude in the way she treated me, she told me that the reason that I’m not getting the result is because of my skin/hair type and they are not responsible. I contacted the owner of the company [redacted] through email, and he asked me to wait for the manager to contact me and give me an answer. [redacted] who was the manager called me after a week and gave me an option of having free treatments with students. I went to the treatments with the students, but they did not do well. [redacted] (one of their technicians) informed me that Lightsheer machine (specifically for light hair people) shouldn’t be used for me. There were two times that I had so much difficulties to get the right machine (1064) and they took me to a different room and wanted to use a different machine on me (due to the reason that other people was using that machine), once I accepted but the second time I told them that I want the other machine and the technician got so mad and took me to the right machine and did my treatments in less than fifteen minutes (she did that between other people’s treatments, and normally takes about an hour). After four months that I stopped the treatments, my hair started growing the same amount that it used to, I talked to many different technicians and doctors about it and they informed me that whoever did the treatments did not do the treatments at a right time (due to hair cycle) and in a right way. I emailed the owner on 09/16/14 and informed him about my dissatisfaction and asked for my money back, today is 09/25/14 and still I did not get any respond.

1. They used a wrong machine for me for a long time. 2. They did not check my hair/skin type never throughout the treatments. 3. They did not ask/informed me about hair cycle 4. I picked the days of the treatments by myself (in reality they had to choose the days due to the hair cycle. 5. I am not satisfy due to the reason that I had no result and I still need to shave and wax. 6. On my free treatments, they did not treat me the same as I had the treatments that I paid for.

Thank you for voicing your concerns, I appreciate your feedback. I am truly sorry that our laser hair reduction treatments have not been as effective as you would like. As I looked into your statements and your situation, here are some of my thoughts:

We are the largest training facility in the country. We train both medical and non medical professionals to be certified laser technicians. We will always practice, teach and promote safety with lasers. This means:

We ALWAYS look at the patient we are about to treat to understand their history and skin type.

Based on the above, we will make a conservative choice on how to treat the client.

We do not guarantee that our laser hair reduction treatments will result in permanent hair removal. There will always be some touch ups necessary. Some people will need more than others.

We do promote that you schedule your treatments based on the hair growth cycle. This will produce the most success.

When you communicated with the manager of the spa in March of 2013, you told her you were unhappy with your laser hair reduction results. She came to me to help her find a solution. We offered you your choice of: unlimited hair reduction in our training facility OR 3 free treatments in a private setting. It is my understanding you opted for the unlimited treatments in the training facility.

You then came back to the manager in April of 2013 to express your displeasure with your treatments on your face and underarms. According the notes in your account, she gladly replaced the treatments you had on the original machine with complimentary treatments on the other machine.

Let me state for the record, that the machine you were originally put on is perfectly safe and acceptable to be used with your hair and skin tone. We leave treatment specifics up to the expert in the room and [redacted] and [redacted] decision to work on that machine vs any other machine was 100% acceptable.

According to my calculations, we have given you over $2,000 of complimentary treatments. Whether it was in the training facility or privately, National Laser Institute has replaced each and every treatment you were unhappy with.
