I am writing in response to the complaint referenced above, received in our office on June 13, 2017, for Mr [redacted] regarding credit inquiries made by Nationwide Bank.In researching Mr [redacted] concern, I find that Nationwide Bank has initiated one credit inquiry on his behalf.The date of that inquiry was September 14, 2015, when an application for an automobile loan was submitted byMr [redacted] There have been no subsequent credit inquiries made by Nationwide Bank.I am attaching a copy of a record of credit inquiries provided by [redacted] using Mr [redacted] information.Although Nationwide Bank has only initiated one inquiry, this record indicates that there have been multipleinquiries submitted by other financial institutions and merchantsNationwide Bank would recommend that Mr[redacted] review this list carefully to ensure that all inquires have been made with his full knowledge and consent.Sincerely,Anne C***SrAnalyst, Member Experience Team [redacted] ###-###-####

Please accept this letter as a formal response to *** ** *** complaint regarding her Monmouth County New Jersey retirement account, administeredby NationwideI welcome this opportunity to address your concerns.Plan detailsPlan name: Monmouth County New JerseyAccount number: ***Understanding the detailsAccording to our records, Nationwide received a Unforeseeable EmergencyDistribution Form for *** *** *** *** *** *** retirementaccount on November 26, 2017. *** *** indicated on the paperwork that shewas requesting funds due to loss of income as a result of illnessRegrettably.the Loss of Income Verification Form submitted by her employer did not clearlyspecify whether the dates of leave were paid or unpaidThat information isrequired in order to calculate loss of incomeIt was explained to *** *** thather employer would need to clarify the leave information before Nationwidecould process her requestA letter from her employer was ultimately submittedon February 20, verifying the dates of unpaid leave.Upon review of the newly submitted documentation, it was discovered that *** would also need to submit verification of her disability award or denialIna letterdated January 23, 2018, *** ***'s doctor stated that she would beapplying for disability benefitsNationwide contacted the member's employer toverify this informationHowever, they were unable to confirm if *** *** wouldbe eligible for disability reimbursement for the time she was on unpaid leaveInorder to process *** ***'s Unforeseeable Emergency Distributionrequest, she will need to confirm her disability award or denial so Nationwidecan include that information in her loss of income calculation.Next stepsI would like to apologize for *** ***'s experience thus farI understand howstressful an illness can be and it was never our intention to add to her worries.As the Plan AdministratorNationwide must comply with the terms of the Planand applicable Internal Revenue Code (IRC) rules and regulationsOnceNationwide receives confirmation of the member's disability award status, wecan move forward with processing her loss of income UnforeseeableEmergency request
