December 10, Dear Representative of the Revdex.com, Our position is clear in the email correspondence between our offices representative Carole E [redacted] and the complainant [redacted] We ask that you review the emails attachedThe x-rays and integral photographs taken, during his initial exam/appointment, are the property of this officeEven if there were benefits paid by a third party like dental insurance, or monies paid out of pocket by the patient, they are for the doctor's knowledge and evaluation/diagnosis of those x-raysHe is not being charged for the actual images he is being charged for the intellectual property of the Doctors and our Center for the physical intellect of the images which under no circumstances at any point in time were [redacted] propertyPete E CEO

September 28, Dear Representative of Revdex.com, Natural Horizons Wellness: Centers received your notice of a complaint about our businessThe complaint was received in our office September 28, 2015; this letter is in response to that complaint.Our office, policy is extremely clear ''All co-pays, co-insurance and deductible amounts are the patient's responsibilityPayment of the estimated portion is expected in full at the time of serviceWe estimate the patients co-pay to the best of our ability using-the information available to usHowever; because we are estimating only, there may be some instances where the patient will have a.balance remaining after their insurance has paid, that will be their responsibilityNo insurance company provides a guarantee of payment and therefore we cannot provide one to the patientPayment will be expected at the time services are renderedPlease let us know if you have further questionsIt is important to realize that if financial problems occur that you maintain contact with our financial staff in order for them to be of possible assistance to you." Every,patient prior to seeing any doctors or staff must sign this document acknowledging this policy [redacted] and her family members were of no exception to this ruleTherefore prior to any service being rendered to her or any of her family members she was notified of her estimated out of pocket expense and required to pay that at the time of service.In regards to her claim of "charges outside of the allowable maximum were charged" is completely inaccurateIn her complaint she states " [redacted] validated our documentation and calculations" is simply not trueOur staff has spent hours upon hours dealing with this unfounded claim of "insurance fraud and billing issues" with endless claims of monies being owed to them [redacted] has reviewed all claims ever billed under this family and has clearly stated they see no errors in the billings of services and.no question on the medical necessity of procedures performedDue to extremely poor oral hygiene, poor eating habits and several other clinical factors of both *** [redacted] and her children, gross decay was a continuous issue we dealt with every:time she and her kids came into our officeWe cannot be held responsible fortheir failure to properly clean their teeth that resulted in recurrent decay and the need to have restorative procedures done.In regards to any claim of monies being owed to them, our records indicate this family owes our practice $7,and this account is in the hands of our collections attorney.If you are in need of any further information please, feel free to contact our office directly.Respectfully,Michelle R.Manager, Dental Insurance Billing and Collections

The dentist at this location told me I had cavities that needed filledAfter receiving several phone calls reminding me to make my appointment for these fillings, I told the dentist he could do the first three, but I would come back for another visit for the next three later After a less than positive experience there with the dentist who did the fillings and also with the billing agent who yelled at me when I told her I had supplemental dental coverage for the fillings, I decided to look for another dentist I chose a family practice my parents and siblings have been going to for many years At the new practice, the second opinion on my x-rays (taken by Natural Horizon's Wellness Center) was that I actually had only one cavity that needed filled, not Natural Horizon's Wellness Center was terribly "mistaken." I have to question their intentionsI do not trust a business that tells me I need more dental work than I actually needI do not trust a business whose billing agent cannot treat her clients with respect

Dear Representative of Revdex.com,Natural Horizons Wellness Centers received your notice of a complaint about our business.  The complaint was received in our office October 7, 2016; this letter is in response to that complaint.This complaint is biased on "Delivery Issues" and states "My husband was...

given an IV infusion that was supposed to contain vitamins and minerals he needed to build him up to get ready to start chemo. He did not receive what we were told in the IV fusions, we do have proof" yet no proof has been supplied in this complaint.  As we are a health care facility we are bound by HIPPA requirements therefore we cannot release his exact medical records and infusion records, but the records have been verified and he did indeed receive all of the IV infusions ordered by his attending physician and his account invoiced correctly for.  The complaint continues and states "When we found out he was not given what we were told we stopped payment on the check for $1014.85" yet this was after three visits, and the stop payment was made within hours of writing the check directly after the fourth date of service was rendered.  The letter received continues "the outcome we want if for this company to remove us from owing them any money" I feel it is clear in this complaint alone this is a scam artist that feels they can go around receiving thousands of dollars in services and just stop payment on a check rendered for the services once provided.  It is unfair situation and nearly impossible position Natural Horizons is in as beyond this information we are unable to defend ourselves any further, without HIPPA release.We have attempted to contact both **. and [redacted], via USPS and telephone unsuccessfully in an attempt to resolve the matter of the stop payment on the check of $1014.85 since September 8, 2016, this is the first complaint of any kind from this patient or his wife and the first any claim of "not receiving what they were told".If you are in need of any further information or are able to secure HIPPA release we are more than willing to have records inspected, please feel free to contact our office directly.Respectfully,Michelle R[redacted]Manager, Dental InsuranceBilling and Collections

The agreement I made with Carole was if I used the x-rays for a second opinion, I would pay for them. I have not used these x-rays in any way and I am not going to use them. As stated in the emails, the x-rays were part of the "free dental exam and consultation - $300 value". Also, your advertisement does not state that there will be an extra charge if the work recommended isn't completely at Natural Horizons.  If this was stated I would have never made the appointment.  To date, I still haven't visited a dentist to do the work and nobody has seen these x-rays, so I have not used the intellectual property either. I simply came in to get the exam which included the x-rays (as stated in your emails). Please honor your advertisement's claims.

After receiving several phone calls reminding me to make my appointment for these fillings, I told the dentist he could do the first three, but I would come back for another visit for the next three later. Natural Horizon's Wellness Center was terribly "mistaken." I have to question their intentions. I do not trust a business that tells me I need more dental work than I actually need. I do not trust a business whose billing agent cannot treat her clients with respect.
