Member agrees that a certifiedletter was received on Nov. 7th by NBD Training Zone.  Thatthis was the first and only proof that member was requesting cancellation onrecord. A messagewas left and was followed with the following email to insure that member wasnotified:  [redacted]On Nov 7, 2014, at 5:59 PM, "[redacted]" <[redacted]> wrote:[redacted],  You may reply back to this email with proof or fax it to [redacted] Attn: [redacted] There are 6 payments remaining on your agreement. Earlytermination of contract according to your contract are as follows. There are 3 ways to cancel your Smart Train agreement whilestill in term. Please respond back to let me know which option you decide.   If proof of move of your primary residents is provided. Proof consist of the following: copy of lease, copy of utility bill (gas & electric, cable etc. ) bank statement. All of which must include the name you have provided NBD with and the new address. New primary address must be located 25 miles or more away from [redacted] NJ 07960. Once approved cancellation fee of 10% of your total contract value is due to stop and cancel all remaining payments. In your case (12 x $99= $1188.00 is your total contract value of which 10%= $118.80 with proof will cancel your agreement.   Canceling fee without proof is = to 20% of your total contract value $1188.00 + the next payment. In your case $237.60 + next payment within 30 days from the date your cancellation request was received.   Medical cancel in which a permanent disability is fully described and confirmed to [redacted], by a physician in the form of Dr. note = to 10% of your total contract value $1188.00.  In your case $118.80  You may email me directly any proof of move that you wish tosubmit.
