additional amount to get her prescription.Upon [redacted]’s check-in on the date of the exam, the patient was presented with our forms with included the Financial Responsibility statement and the Refraction Policy. It was explained to the patient what the refraction policy was and the fee for the refraction of $35.00 for the prescription which was a fee for additional services in addition to the exam. Furthermore upon check-in the patient was provided the choice that she did not need to sign the refraction policy and pay if she chose not to have a prescription written.It is the standard of the industry that the refraction fee is a charge for an additional service and accordingly is billed as a separate service to the patient’s insurance. In this particular case, we further determined that the patient’s insurance plan was providing a separate benefit for the refraction fee of [redacted]’s in addition to benefit paid for the exam. Accordingly, we are reimbursing the payment of the $35.00 to [redacted]. The payment is being processed and will be sent to the address we have on record on this patient. We are sorry we did not meet [redacted] service expectations. Due to regard to our HIPAA policies and procedures, we are unable to provide you with signed copies of documents by the patient which included the refraction policy. I have read the statement from the company. I was informed that for my regular checkup that my insurance didn't pay for my prescription and that was the fee I was being charged. But I am satisfied with the reimbursement. thank you for your help.
