New Chapel EMS is responding to complaint I.D. [redacted]. After looking into this I will highlight some events in (complaint I.D. [redacted]) statement of problem that are inaccurate.
1. (complaint I.D. [redacted]) stated he was transported multiple times by New Chapel. Which is inaccurate due from...

New Chapel has only been in contact twice. First contact by ambulance crews ended in signature of release meaning he declined treatment and transport. No insurance information was gathered being he was not transported and we only charge if transported. On the signature of release he supplied a date of birth being on 12-6-1965.
On second contact by ambulance crew (complaint I.D. [redacted]) was transported and according to EMS run report he was not cooperative and agitated. Therefore he did not supply any insurance information and supplied a different date of birth 01-26-1965 than first contact, Billing tried to see if(complaint I.D. [redacted]) was eligible for Medicaid and date of birth entered from second contact did not match to (complaint I.D. [redacted]). Billing was not aware of first contact being (complaint I.D. [redacted]) was not billed being signature of release until contacted by myself to look into this matter for Revdex.com.
2. In Statement of the Problem (complaint I.D. [redacted]) stated he received bill from New Chapel EMS and mailed it back with no payment but supplied insurance information. This is incorrect after speaking to billing. They never received any insurance information through the mail from (complaint I.D. [redacted]). New Chapel EMS nor Billing have had any contact with(complaint I.D. [redacted]) since his second contact on July of 2013 until 09-03-2015 he contacted billing stating he was not paying the bill and wanted it taken off his credit report, Billing explained even if you supplied insurance information it would be too late to bill insurance being there are deadlines to bill 6-12 months from initial date of transport(note! (complaint I.D. [redacted]) received multiple bills within the deadline to pay, supply insurance info, or contact billing for other options). Mr. Mitchell became uncooperative and agitated hanging up the phone.
