on front teeth over a year ago at another office and recently these fillings come offPatient had been to several dentists within the last years prior to our officeExam and x-rays were done on the same day and treatment options were discussedDr*** discussed risks, benefits and estimation of copay with patientPatient chose not to forgo with treatment on today visit because of finance limitationPatient opted to wait on the treatment until August when her insurance will pick up some portion of the treatment. On august 18, 2014, patient presented at our office for filling of teeth # and #Patient was giving an estimation of her insurance carrier’s ESTIMATED PORTION of payment and her ESTIMATION PORTION of share of costPatient was informed that she is fully responsible for all fees not cover by her dental insuranceShe signed the above acknowledgement and proceeded with the treatmentServices were rendered and patient was very happy with the esthetic and functional result of Dr***’s treatmentAs a courtesy to patient, we help patient submitted treatment claim the insurance companyThe total cost of treatment was $Insurance paid $on 9/9/Patient paid $76.10, that left patient a balance of $71.10.We send patient a statement of her remaining balancePatient did not responseAgain we attempted more times, still no payment madeSince patient was never responded, we send patient account to collection agency on February Again, patient never response to collection agency letters and notices. Patient contact our office on August 4, 2015, demanding answer to why she was sending to collectionPatient was extremely rude, yelled over the phone, and made numerous threatsOur office Manager, *** ***, stated to patients that we can schedule an appointment with patient to go over her accountPatient demanded immediate answer and the hung upPatient then made over annoying and harassing calls in the next few days and refused to make appointment to discuss her accountOur front offices staff made numerous attempts to explain to patient the followings. 1. Patient insurance policy is a contact between patient, her employer, and the insurance companyOur office is not a party of that contractOur relationship is with the patient and not the insurance companyKnowledge of benefit amount as well as limitations, exclusions, waiting period, etc…is entirely patient’s responsibility.2. As a courtesy to patient, we have called insurance company to help patient obtained break down of her benefitsEverything insurance provided to our office is just estimationTherefore, treatment cost provided by our office is an estimation as wellThis is true of all dental insurances and at all dental officesReceiving our services indicates patient acceptance of responsibility to pay regardless of estimateWe have a copy of patient signature of the acknowledgement prior to treatment.3. Patient have a copy of her insurance policy handbookAll fees are just an estimation until services were rendered and claims were submitted for paymentPatient insurance company then determined the pay amount based on the limitations constructed for patient policyAn EOB (explanation of benefit) were also sent to patient from the insurance company after we submitted the claimsInsurance company sent this document to patient prior to sending the payment check to our officeThe EOB showed patient how much insurance company pay for the submitted treatment and the amount of patient responsibility.4. Therefore, patient had knowledge of what her insurance paid and her remaining balance prior to September 9, 2014, over months before we sent her account to collection.5. Our staff also offered monthly payments to help patient pay off her $account balancePatient declined offer. Per patient requested, on September 2, 2015, we sent copy of patient account information included signed treatment plan, signed filling consent, and a letter explained why her account was sent to collectionPatient later called back and stated she never received anything from usAs patient requested, we mailed her a second copy of all the document again. I am rejecting this response because: The information provided by Dr*** is incorrect. When I initially came to Dr*** who ran an ad in a local magazine that indicated she had an easy payment program for her services, an exam and x-rays were performed as she stated, however, I did not willingly forgo treatment because of financial limitations, I was rejected further services until August BY Dr*** because of a 2-year waiting period with the insurance company that she informed me ofI gave her the benefit of the doubt because she had already performed the exam and x-rays and decided to return to her in August to perform the servicesWhen I went there they asked me for my copayment and they informed me that the rest would be covered by the insuranceThere was no indication prior to leaving that day, after services were rendered, that there was any remaining balance or else it would have been paid as I had already paid my copayment of $as she had stated in her responseIf there was a remaining balance of $due to Dr***, no one made this known prior to me leaving the office or why there was a balance remaining after I had already paidMONTHS later I received a bill from her. Of course I called the office to obtain information regarding the bill and was told that it was some sort of mistake and it would be taken care of but they never called me back as they stated they would do. Only to receive a collection notice months later in regards to the same bill which was not taken care of as I had been previously told my the office when I called. I called the collection company regarding the bill and I was informed that I couldn't pay it because they didn't have the bill in their possession and they didn't have the proper paperwork to accept the payment and this would probably be dismissedI was extremely dissatisfied with the bill going to collections and being reported to my credit report so I called to request a copy of the invoice of the second visit services renderedWhen I called the office, they were very rude and refused to send me the informationI was finally provided the invoice about two weeks ago which showed a copy of the invoice from the day services were rendered which show a ZERO balance when I left, yet she still claims I owe her officeI was very dissatisfied with the lack of information I was getting from the office, and disputed the charge to the credit bureau but they were not able to take it off because the doctor claims I still owe her money although to this day, I have not been provided with a breakdown of the remaining balance or why it wasn't collected at the time of serviceDr*** is trying to indicate that she did me some sort of favor in regards to this matter and that is just untrueNo one ever called to offer me any type of resolution of payment or any payment plan as she statedShe has never called me herself to explain why the funds were not collected at the time of this visit which I have requested for more than a year of this information which again, still has not been provided to me All she and her office has done is be extremely unprofessional regarding this situation and create confusion regarding what they are charging me forShe is unethical in her practice and is also providing advertisement indicating she has an easy payment program that she does not honor. Review: Dr. [redacted] and I had an agreement that she would bill my insurance and accept that payment, as payment in full. I have been seeing her for many years and most recently she performed a root canal and made arrangements for me to pay for the porcelain crown only, $600.00. At the time of service I paid $300.00 (down pymt). At this time an impression was taken of my tooth for the future crown. On this day, Dr. [redacted]'s office had me give them a copy of my debit card so that on the next appt when the crown was to be put on, they would charge my card for $300.00. I received a call from Dr. [redacted]'s office stating that the impression was not done correctly and I needed to return to have another one done. I told the assistant that I would call to make another appt for that procedure and also instructed her NOT to take the payment yet, since we still had time before the actual crown. She stated that she would indeed do this. My Bank (Chase) informed me that I was overdrawn in my acct and it was due to Dr. [redacted]'s office withdrawing the $303.00. I called their office and spoke to another assistant and I asked her why they did this, since I had NEVER had the procedure done and I asked her not to make any charges on my Bank card, as I did not feel comfortable with this. She stated that she would to with Dr. [redacted] and get back to me. She called back and stated that Dr. [redacted] would not reverse the charges. Later on, I called the office and advised the girl that I would not be returning to New Dental Images and since the crown was never ordered, I would not be returning as a patient. Dr. [redacted]'s office owes me $606.50 (porcelain crown charge). She told me that I would not be paying out of pocket for the root canal (only for the cost of the porcelain crown). Since complaining to her office, I suddenly received a "dummy bill" in today's mail with fraudulant charges that add up to $918.35.Desired Settlement: I want to be reimbured the $606.50 and the bogus charges be remove the charges that she just created.

Patient had treatment done at our office on 09/05/2013. Patient expected portion was $606.50. Patient paid $303.25 on day of service. Patient made a financial arrangement with a copy of her credit card. Agreement was made on 09/26/2013 for the remaining amount of $303.25. On 09/23/2013, patient called the office, spoke to the office manager and stated that she's having car issues and request office not to run her credit card on the initial agreed date on 09/26/2013 and postpone payment to 10/16/2013. Patient did not follow through with her agreement and made no payment. Office sent her a bill on 10/18/2013. Office gave patient a courtesy month from her agreed date to run the payment. Since patient has not responded within that month, office ran her credit card on file the amount that she owed according to our agreement of, $303.25 on 11/20/2013.

I do not agree whatsoever with Dr. [redacted]. She handled this very poorly. I cancelled my appointment due to an unforseen financial setback. My vehicle was in the shop being repaired. I called prior to the appointment for impression to be taken and sent to the lab. She ignored my request to wait and do not charge my bank card. And, she lied to the Revdex.com stating I owed her $900. for services rendered. As we agreed she would accept insurance payment only. She made an invoice after this incident. She collected the $303. I retrieved it back by filing a claim. She owes me $303. I do not have the porcelain crowns

On January 10, 2014 at 8:00am [redacted] Came into our office to get her refund of $536.50.

Review: I went to this facility in the beginning of 2014 when they were at their previous location and I am sure nothing has changed with their new location based on the research of reviews I have conducted. I went there to have a broken tooth fixed. They did the exam and the assistant told the doctor that the insurance would not cover the services that day because I had already needed a broken tooth fixed less than two years ago. So I informed her that I would go ahead and pay the $275 today and put the rest on credit which they had offered to me and advertise for. She came back with news that the doctor did not approve giving me the credit and that I would have to come back in August to have the work done. So I went back in August and they told me my copayment was $76 which I paid and they verified that my dental insurance would cover the remaining amount just as they were already paid for my first visit for the exam and x-rays. In December of 2014, I received a phone call from the office telling me I owed then $71. After speaking to the receptionist about how I had already paid and was never given any sort of bill at the time she told me that they actually forgot to bill me but not to worry and that the doctor would not make me pay because it was their fault. That was the last I heard of it until February when I received a collection notice for $75. I called multiple times to ask what this was in regards to and that the issue should have already been resolved. Each time I called to get clarification and a resolution I was hung up on and talked to in a very rude manner. 2 weeks ago I received paperwork that showed a $0 balance on my part and I asked for the documentation from my previous visits and was told that "they don't have to send me anything". They have poor ethics, poor handling of billing issues and act like they are above the law with no regard for patients. I am not the only one who feels this office is scamming people into paying money.Desired Settlement: Because of the confusion and misinformation given to me by the receptionist at the front desk in regards to my billing issue and lack of customer service as well as lack of honesty and a true explanation of the payment amount due after I received services at their office of which I was told was taken care of, I would like them to rescind the collection they put against me for $75. I have called countless times to resolve this issue and was never called back as they claimed they would.
