On September [redacted] 2016, member Gerald C [redacted] , signed a membership agreement that indicated that the agreement was for a minimum of twelve months, at which time membership may be terminated at any time with forty five days notice to EquinoxWhile within the one year obligation, the membership can be cancelled with forty five days' notice for relocation(member must provide proof that they have relocated more than miles from an Equinox facility), job loss (member must provide proof of job loss), and medical (medical note is required and account will be cancelled immediately)
