seen as a new patient on September 8, The patient provided us insurance information.Patient did pay $on January 4, 2017.Insurance claim was closed on January 19, As of today I have not beenpaid by insurance companyMy office did receive a denial (EOB) on January 19, from Blue Cross Blue Shied.On April 28, my office attempted to call regarding account.May 11, patient came into office with a copy of their statement and inquiredabout their balance in the amount of $this includes billing & finance feesMy employee verbally verified insurance with patient and was told it was correctHe did not at the time provide her with a phyical insurance cardMy employee did write on their copy of statement to "disregard - claim resubmitted 5-11-17.On May 22, my office left a message regarding account.My employee was not able to resubmit to updated insurance company, due to the fact, ID # *** never returned message.Between May 11, and February 5, my office has not had any response from ID ***.Statement from my office continued every month and mailed to the address on fileJanuary 24, Account was submitted to collectionsFebruary 5, letter was sent from Revdex.com and received on February 8, My office has made every attempt to resolve their outstanding balance.Regards,DrWilliam CG***, DDS
