Dear [redacted] , As we discussed today, the following is a statement in response to the issue referenced above: Ms [redacted] has raised a concern about a thirty dollar co-pay made on 2-9-in regards to her daughter's evaluation and care at our ophthalmology medical eye care office This same amount ($30.00) was voided as a charge to the credit card on the same day of service (2-9-15) A copy of this voided transaction was faxed to Ms [redacted] on 2-11-and she acknowledged receiving the fax on that day (2-11-15) There should be no remaining issue in this regard and this matter is believed to be resolved Ms [redacted] is welcome to contact our office at any time if needed to further discuss any questions As we take great pride in providing high quality medical eye care and evaluations, we appreciate Ms [redacted] concerns and wish to add her among our many very satisfied patient families Thank you

Review: MY DAUGHTER WENT TO THIS OFFICE FOR AN EYE CHECK FOR GLASSES. MY DAUGHTER DIDN'T KNOW BETTER SO SHE GAVE THEM 2 INSURANCE CARD, HER MEDICAL INSURANCE CARD [redacted] AND THE ANTHEM BC VISION INSURANCE CARD. THEY CHARGED HER THE $30 COPAY AND ALSO CHARGED $320 FOR GLASSES AND I THINK FOR THE EXAM. [redacted] AT THE OFFICE SAID THAT SHE COULD NOT FIND MY DAUGHTER WITH THE VISON INSURANCE, AND MY DAUGHTER CAME WITH A ROOM MATE FROM UCI, THEY HAVE THE SAME EXACT INSURANCE, ANYWAY, MY DAUGHTER PAID, BUT THEN AFTER ALL IS SAID AND DONE, MY DAUGHTER CALLED ME AND TOLD ME OF WHAT HAPPENED, SO I CALLED NEWPORT EYE PHYSICIANS, AND SPOKE WITH [redacted], AND I TOLD HER THAT MY DAUGHTER HAS VISION INSURANCE AND SHOULD NOT HAVE BEEN CHARGED THE $320, AND THEN I ALSO QUESTIONED HER ABOUT CHARGING MY DAUGHTER THE $30 COPAY, THEN SHE SAID " OH BECAUSE YOUR DAUGHTER GAVE ME THE [redacted] CARD SO WE WILL TREAT HER [redacted] INSURANCE AS SECONDARY" I TOLD [redacted], SHE CAN'T DO THAT BECAUSE THEN BILLING [redacted] IS FRAUD, I KNOW, BECAUSE I AM A MEDICAL BILLER, IT IS FOR AN EYE EXAM FOR GLASSES, IT IS NOT EN EYE EXAM BECAUSE OF INJURY TO THE EYE. SO SHE GAVE THE PHONE# OF THEIR BILLING OFFICE AND I CALLED THEM AND SPOKE WITH ROB, AND HE SAID THAT I WAS CORRECT BECAUSE [redacted] IS A MEDICAL INSURANCE, I CALLED BACK [redacted] AND SHE SAID THAT SHE WILL VOID THAT $320 TRANSACTION AND CHARGED MY DAUGHTER IS $202.00, WHICH IS STILL A LITTLE BIT HIGH, BECAUSE MY DAUGHTER HAS VISION INSURANCE, AND THEN I TOLD HER THAT SHE NEEDS TO CREDIT BACK THAT $30.00 COPAY BECAUSE MY DAUGHTER SHOULD NOT OWE THAT, I CALLED AND SPOKE WITH GLORIA AND SHE SAID THAT SHE WILL VOID THE CHARGE, THIS WAS MONDAY, CALLED HER YESTERDAY AND TOLD HER THAT THE CREDIT WAS NOT IN MY BANK, SHE SAID THAT SHE VOIDED THE CHARGE, BUT SHE WAS LYING, BECAUSE TODAY, I SAW THE $30.00 CHARGE ON MY ACCT. I HAVE CALLED THE THE MEDICAL INSRUANCE AND TOLD THEM OF WHAT HAPPENED AND THEY SAID THAT THEY WILL NOT PROCESS THE CLAIM, AND NEWPORT EYE PHYSICIANS NEED TO REFUND US THE $30.00Desired Settlement: I WANT REFUND OF THE COPAY PAID AND IF MY DAUGHTER PAID MORE ON THE EXAM AND GLASSES. BECAUSE I KNOW HER VISION COVERS EYE EXAM AND GLASSES.
