Ms. [redacted] Child was enrolled and started on January,04.2016. Her last day was February 02,2016Ms. [redacted] income falls below the proverty guidelines, therefore she was only responsible for the co-pay, which was $24.00 a week. Ms. [redacted] paid a total of $168.00 ofr the seven weeks her daughter was enrolled, not the two she claims. The biological father had another chil in the center for years, and he asked me if I remembered him, and that's how [redacted] attended.Ms. [redacted] gave the infant staff a daily schedule when she wanted heer to be fed. Ms. [redacted] worked directly across the street at [redacted] ***, at [redacted] Road. The day care is located at [redacted] Road. Ms. [redacted] would come over during the day to breastfeed. On occasion she was unable to come and would have staff give the baby a bottle. Breast feed generally have a stool with each feeding, she was changed wheneveer needed. Stools were generally very soft. The state of Ohio does not allow any day care to wash soiled clothing. We can only rinse the clothing and place it in a plastic bag. Ms. [redacted] did have a eight ounce bottle of milk, that fell from the freezer, I don't know how she placed eleven ounces in the bottle. Ms. [redacted] was informed of the incident. Ms. B [redacted] , also told her not to use the milk, because even though the bottle didn't leak, glass fragments may havr brokren off inside the bottle. The day care is staff appropritely for the number of chidren. We were always in ratio in the infant room. It would be nice to hire additional staff. Private pay parents that are paying $200.00 per day can request smaller ratio's. It is true that another infant touched her child's head. There was no poking involved. The incident was developmentally appopriately, tha's how infants learn. Staff was present at the time of the incident. Staff does not yell at the children and I have never been in the building when Ms. [redacted] was here, therefore she could have never heard any conversation between me and my staff. There is no truth in that statement. I did not want her to fabricate billing, I wanted her to sign for all the weeks her child attended. I took Ms, [redacted] the attendance sheets and the daily sign in sheets to verify that these were indeed her signatures. She confirmed that they were her signatures and agreed to pay her co-pays for seven weeks, not two weeks that she fabricated. Ms. [redacted] told me or staff that she was removing her child, after she signed her manuel forms I told her not to return the child. This conversation did not take place in the center and she never had additional contact with staff to have any unpleasant conversations. There is no child abuse taking place. There no videos and mother never asked.
