Please be advised that *** *** signed a one year contract with Next Level formerly known as (*** ** ***) on 07/26/Her contract start date was 08/06/thru 08/06/which automatically renews if not cancelled within the terms of the contractAccording to the terms of the contract a
day cancellation letter via certified mail must be received prior to the completion date of the contractAt know time did we receive any such noticeIn addition Mrs*** continued to train and make her schedule monthly payments in the months of July & Aug of her renewed contractIn September she requested a freeze to her contract do to medical reasons and would resume her training in Oct On Oct 1, I reached out to Mrs*** via email to inform her that her credit card was declined, at which point she advised that her identity had been compromised and that she would stop by later that week to provide us with an updated method of paymentOn Oct 20,Mrs*** via email advised us that she had every intention on coming in with her new credit card even though she would not be able to come in for workouts because of her schedule and health related problemsShe also, advised that she had consulted with her neighbor about auto-renewal contracts, she advised that she was not aware of the auto-renew clause which is boldly stated on the first page of her signed contractPlease be advise if copies of her signed contract and her email communication with us is requiredThanking you in advance*** *** Manager
