We have a copy of the estimate (treatment plan) in your chart dated 3/26/15. You absolutely would have received a copy of the estimate and my office manager told you that we would be collecting a copay and submit this to your insurance. According to the estimate we collected $(not $30) as a copay for the filling, NOT a deductible. We knew that you had no deductible for fillings from your benefits that were faxed to our office on 3/19/15. We collected $as a copay, explaining to you that we would submit the claim to insurance and bill the remainder to you. I am obligated by my contract with your dental insurance to collect the amount stated on the explanation of benefits

Mr*** presented to the office on 3/25/complaining of discomfort on the upper left side. Appropriate radiographs and an exam determined he had a large cavity under and around an old silver filling. We treatment planned the filling. At that point we reviewed with Mr***
that his dental insurance plan downgrades posterior silver fillings and will only pay a benefit for a silver filling. He was informed by both my front office manager and myself personally that he would be responisible for the out of pocket difference between the cost of the silver and composite filling. Mr.*** was even told that we do not offer silver fillings and I mentioned that I don't even have Amalgam in the office. We estimated his out of pocket expense to be $153.29. We collected a cash payment of $for his copayment and informed him we would send a bill after we received payment from the insurance company. There is no deductible for his dental plan. His payment of $is an acknowledgement that he had a copay and it was not covering a deductible. My front desk manager had his benefits faxed over on 3/19/15, so we did in fact have his benefit information and were able to inform him as we did about what is and isn't covered under this dental plan. According to *** ***, we received an explanation of benefits with the payment that the patient is responsible for $minus his office copay of $35, left him a balance of $228.00, which we sent multiple statements for. Mr*** would have received the same explanation of benefits from his insurance company. I have a contract with his insurance company and I have to bill in accordance with my contract with *** ***. Mr*** had subsequent appointments scheduled in my office on 4/18/and 2/20/15, which were broken appointments that he did not show up for. I personally followed up on 6/18/and 7/1/with phone calls and left messages stating that I would be happy to review the Explanation of Benefits and billing. I made multiple attempts to address the patient directly and settle the dispute with him. We have been sending statements to him for approximately months, so some of the balance is due to finance charges on the account. We sent certified mail on 9-9-15, which was signed for and accepted. I feel that I provided a service and he was informed about and knew there would be an out of pocket expense for his dental treatment. I am following my contract with his insurance and billing as I am told by insurance for the services provided

I am rejecting this response because:Because I was never told a price before they did the work sitting in the chair. I didn't know a price till I got a bill in the mail. Second I have no copay for fillings in my insurance, so why did they collect it. Why because the girl at the desk never checked my insurance like she said she would. All her responses are not answering any of the issues either. Whom ever is answering is avoiding answering truthfully!!!
