Price quoted for testing was $417.50 and price charged without authorization has been billed at over $1600. Prior to scheduling an appointment for a test as recommended by our doctor we requested a quote and asked to confirm our insurance was accepted. Our Insurance was accepted and the quote for this test was $417.50 was confirmed by email. After we paid the $417.50 we are now being billed for over $1600 for this one test. We were never notified that the charges would increase. We have contacted their billing department and have asked for an explanation as to why the charges have increased so much without any prior authorization or explanation. Initially the billing department said they would investigate and would get back to us. After several attempts to contact them by email and phone we are not receiving any further information except for the bills. Desired SettlementWe have requested the charges for this service be adjusted to the quoted amount of $417.50.Business Response This patient was quoted and estimated minimum cost for the evaluation at"417.50". Please understand ESTIMATED MINIMUM. We also advised the patient to call and verify they own benefits as we do it as a courtesy. Benefits quoted is not a guarantee of payment. All information was emailed to the patient. I see on several occasions the patient in fact did receive a response from our billing department. All of the points of contact are documented in the billing system and via email. We have no control of how insurance companies make their final determination. The balance due is part of the patients deductible. I show a balance due of $356, not 1600.

They continue to try and collect a $210 balance and now threaten a collections tactic.Account # [redacted]Patient is [redacted]We have no medical insurance and have been prepaying for all visits since the first visit.5.20.14 -first visit $500 collected before services. Balance zero.Then I start receiving invoices; 1st one was $80, then the balance was $210 and has been since. We have stopped office visits and they now threaten me with collections. I tried to work with [redacted] to resolve but have not been successful. She claims this balance is from the initial visit. We paid $500 and I assume they want $710.Desired SettlementZero balance the account. We do not owe $210.Business Response This is not an accurate balance. [redacted] have not processed these charges. All billing statements are auto generated. The "collections threat" is auto generated after the max billing cycle. This is an insurance problem with all of our [redacted] patients. [redacted] is requesting notes on EVERY patient. As you can imagine this is a lot of work. We have been working with our attorney to resolve this ongoing issue. Disregard ALL balances. There is a zero balance on this account. We are advising all patients to contact [redacted] directly to complain.

Scheduling an appointment and failure to permit communication with office manager.I have tried to get my son an appointment to see [redacted] for several months. I have called but no one answered the phone or returned my calls at the Raleigh office. I called the Chapel Hill office and my call was returned. I explained the trouble I was having getting in touch with the Raleigh office. The Chapel Hill receptionist [redacted]. I explained my son was seen in the past to me. I asked that she schedule my son for an appointment. [redacted] Primary care sent a referral three times to [redacted] at the Raleigh office. [redacted] said she never received the referrals. I called [redacted] and the nurse said she has fax confirmation of each time the referral was sent. I told [redacted] this and this and she said tell them to send it again; by this time about six weeks had past. I called several times and left messages and frustrated I called **-[redacted] at the Chapel Hill office who set me up with an appointment in Chapel Hill. [redacted] called me back and [redacted] would not be there that day and he did not have any upcoming open appointments. [redacted] said said she would be going on vacation that week and her relief would be able to help me. I called a few days later back and her relief scheduled the appointment for the Raleigh office July XX XXXX. On July 25, 2016 I received a call from [redacted] who said the [redacted] had expired and I offered to pay for the visit ( as it is important that my child not miss a full day of school) and [redacted] would reimburse me [redacted] said [redacted] would not reimburse me. that it would be best to cancel the appointment. I contacted [redacted] and the [redacted] card was reinstated July 26, 2016 and I contacted Shannon left her a voicemail and **-[redacted]. [redacted] who contacted [redacted] and verified the insurance was reinstated and active coverage from 7/1/2016 to 6/30/2017 . **-[redacted] would not scheduled an appointment as she did not see it in [redacted] which is what the providers use to view patient coverage, but she verified by telephone with the insurance provider. I feel I have been courteous and patient getting the run around from the receptionist(s) regarding scheduling an appointment. It seeming to me that I was being intentionally blocked from receiving services and that the delays directly related to insurance to expire as it appeared to be one issue after another for service.Desired SettlementI would like to be contacted by the office manager.I would like my son schedule and seen by Dr. [redacted] before he starts school on August XX XXXXA request was made to speak to the office manager and it was not honored
